THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show in:
* item 19a (Mailing Address After Separation) his address as XXXX Commonwealth Place, Virginia Beach, VA, vice the street address in Hampton, VA
* item 19b (Nearest Relative) his spouse, Linda H______ and the address listed above vice Samantha H_______ and a different street address in Virginia Beach
2. The applicant states no errors were made; he would just like his file updated.
3. The applicant provides his DD Form 214.
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Regular Army on 16 November 2006. His last unit of assignment was with the 119th Transfer Cargo Company, Rear Detachment, Fort Story, VA.
2. He was honorably released from active duty on 22 April 2012 and he was transferred to the 547th Transportation Company, Washington, DC, DC Army National Guard.
3. The DD Form 214 he was issued shows the following entries in:
* item 19a - X Beverly Street, Hampton, VA
* item 19b - Samantha H______, XXXX Adelia Drive, Virginia Beach, VA
4. His Enlisted Record Brief, dated 14 February 2012, shows he was single at that time and had one adult dependent.
5. Army Regulation 635-5 (Separation Documents) at the time established the standardized policy for preparing and distributing the DD Form 214. The DD Form 214 is prepared for all personnel at the time of their retirement, discharge, or release from active duty. The DD Form 214 is a synopsis of the Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement, or discharge. The mailing address after separation and the nearest relative/address listed in items 19a and 19b of the DD Form 214 are provided by the Soldier.
DISCUSSION AND CONCLUSIONS:
With regard to correcting the applicant's DD Form 214 to show his current address and his spouse's name and address as his nearest relative, it appears he provided the information on the DD Form 214 at the time of his release from active duty as he stated no errors were made and he just wanted his DD Form 214 updated. The DD Form 214 provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement, or discharge. There are no provisions to change or update entries on the DD Form 214 when conditions may have changed after a Soldier's discharge. Therefore, there is no basis for granting the applicant's requested relief.
