THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that her DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show in item 12b that her separation date is 20140621. 

2.  The applicant states that her DD Form 214 incorrectly shows that her separation date in item 12b was 20130621 instead of 20140621.

3.  The applicant provides copies of her DD Form 214 and separation orders dated 15 May 2014.

CONSIDERATION OF EVIDENCE:

1.  The applicant was serving  as a staff sergeant in a United States Army Reserve (USAR) troop program unit at Fort Totten, New York when she was ordered to active duty in support of Operation Enduring Freedom on 19 June 2013.

2.  She deployed to Afghanistan during the period 20130807 – 20140505 and was returned to Fort Dix, New Jersey where she was honorably released from active duty (REFRAD) on 21 June 2014 after having served 1 year and 3 days of active service.

3.  However, her DD Form 214 issued at the time of her REFRAD reflects in item 12b that she was separated on “2013  06  21”.

DISCUSSION AND CONCLUSIONS:

1.  The applicant entered active duty on 20130619 and served 1 year and 3 days of active service.  She was honorably REFRAD on 20140621.  However, her DD Form 214 issued at the time of her REFRAD shows she was separated on 20130621 instead of 20140621.

2.  Inasmuch as it appears that an administrative error occurred at the time her DD Form 214 was prepared, it would be appropriate to correct her DD Form 214 to show in item 12b that she was separated on 20140621.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by showing in item 12b of her DD Form 214 that she was separated on 20140621 instead of 20130621.
