BOARD DATE: 21 April 2015
DOCKET NUMBER: AR20140016083
THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:
1. Application for correction of military records (with supporting documents provided, if any).
2. Military Personnel Records and advisory opinions (if any).
THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show her first name spelled as "J_____N" instead of "J______R."
2. The applicant states her record is in error.
3. The applicant provides:
* DD Form 4/1 (Enlistment/Reenlistment Document - Armed Forces of the United States)
* Marriage Certificate
* License and Certificate of Marriage
* Certificate of Identification for Child of Foreign Birth
* Social Security Card
* North Dakota Driver License
* Uniform Services Identification Card
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the U.S. Army Reserve on 8 October 2013. Her DD Form 4/1 lists her first name as she requests, "J_____N."
2. On 4 February 2014, she entered active duty. On 26 July 2014, she was honorably released from active duty training. The DD Form 214 she was issued lists the spelling of her first name as "J______R."
3. Her records contain numerous personnel documents which lists her first name as she requests. She authenticated many of these forms, as required, by placing her signature in the appropriate place using the spelling of her first name as she requests.
4. The applicant provides a copy of her driver license and numerous other forms of identification which lists her first name as she requests.
DISCUSSION AND CONCLUSIONS:
The evidence of record shows the applicant's spelling of her first name as "J_____N" upon enlistment. This spelling of her first name is consistent with the spelling of her first name on numerous documents contained in her official military records throughout her military service. She authenticated several forms which contain this spelling of her first name. It appears her first name was inadvertently misspelled on her DD Form 214 at the time of her release from active duty training. Therefore, she is entitled to correction of her DD Form 214 to show the spelling of her first name as "J_____N."
