THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant, the widow of a former service member (FSM), requests correction of the FSM's DD Form 214 (Report of Separation from the Armed Forces of the United States) as follows:
* item 14 (Selective Service Local Board Number) - change city to Williamson
* item 43 (Name and Address of Last Civilian Employer) - change the name of his last employer to Island Creek Coal Company
2. The applicant states they appear to be typographical errors.
3. The applicant provides the FSM's:
* DD Form 214
* death certificate
CONSIDERATION OF EVIDENCE:
1. The FSM's complete military records are not available to the Board for review. A fire destroyed approximately 18 million service members records at the National Personnel Records Center in 1973. It is believed that much of the FSMs records were lost or destroyed in that fire. However, there were sufficient documents remaining in a reconstructed record for the Board to conduct a fair and impartial review of this case.
2. A DD Form 62 (Certificate of Acceptability), dated 2 February 1951, shows the FSM was found to be acceptable for induction. The local Selective Service Board address is shown as Williamson, West Virginia.
3. On 7 November 1951, the FSM was inducted into the Army of the United States and placed on active duty. His induction paperwork was not available for review. The name and address of his last employer prior to induction was not available in the records available.
4. On 12 January 1952, he was discharged. His DD Form 214 contains the following information:
* item 14 - shows the city of the local Selective Service Board as Silliamson
* item 43 - shows his last employer as the Allen Creek Coal Company
5. He died on 14 January 2014.
DISCUSSION AND CONCLUSIONS:
1. The DD Form 62 clearly shows the FSM's local Selective Service Board was located in Williamson, WV. Therefore, item 14 should be corrected to show Williamson instead of Silliamson.
2. There is no evidence in the available records and the applicant did not provide any evidence showing the FSM's last employer prior to his induction. Therefore, there is insufficient evidence to make a correction to item 43 of the FSM's
DD Form 214.

BOARD DETERMINATION/RECOMMENDATION:
1. The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending the FSM's DD Form 214 with an effective separation date of
12 January 1952 by correcting item 14 as follows:
* delete: Silliamson
* add: Williamson
2. The Board further determined that the evidence presented was insufficient to warrant a portion of the requested relief. As a result, the Board recommends denial of so much of the application that pertains to correction of item 43 of his
DD Form 214.
