THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of the entry shown in item 7b (Home of Record) of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show his home of record (HOR) is Chicago, IL  60607.

2.  The applicant states:

   a.  he entered active duty from the Chicago Military Entrance Processing Station (MEPS), in Des Plaines, IL on an immigrant visa; and 
   
   b.  because he entered the Army through a pilot program, his recruiter mentioned his HOR could only be changed at his discharge as a US citizen.

3.  The applicant provides copies of his:

* DD Form 4/1 (Enlistment/Reenlistment Document – Armed Forces of the United States)
* USMEPCOM Form 68-3A-E (Request for Examination)
* US Office of Personnel Management (Transmittal)
* USCIS (US Citizenship and Immigration Services).Form G-845 (Document Verification Request)

CONSIDERATION OF EVIDENCE:

1.  The applicant's official military personnel file (OMPF) includes a DD Form 4/1 and DD Form 1966/1 (Record of Military Process – Armed Forces of the United States) completed during his initial enlistment process.  The HOR entries listed Items 3 and 4 (HOR) on both of these documents, respectively, contains a "Dearborn, Wayne, MI address.  His security questionnaire shows he lived in Dearborn, MI from June 2009 to the present.  He enlisted in the Army on 21 April 2010.

2.  On 20 March 2013, the applicant was honorably discharged from active duty.  The DD Form 214 issued at that time shows he completed 2 years and 11 months of creditable active service.

3.  Item 7b of his DD Form 214 contains a Dearborn, Wayne, MI address.  

4.  Army Regulation 635-5 (Separation Documents) at the time provided detailed instructions for completing separation documents, including the DD Form 214.  It states to enter the HOR at the time of entry on active duty in item 7b.

5.  The applicant provides copies of his DD Form 4/1 and USMEPCOM FORM 680-3A-E, both showing his HOR as a Dearborn, Wayne, MI address.  There are no documents in his OMPF showing his HOR as Chicago, IL.

DISCUSSION AND CONCLUSIONS:

1.  The applicant contends his record should be corrected to show his HOR as Chicago, IL.

2.  The evidence of record confirms the applicant's HOR was identified as "Dearborn, Wayne, MI" during his enlistment processing as shown on his DD Form 4 and DD Form 1966/1 contained in his military record.  Accordingly, his HOR is correctly documented throughout his military record and there is no basis upon which to grant the requested relief.
