IN THE CASE OF:
BOARD DATE: 7 October 2014
DOCKET NUMBER: AR20140004306
THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:
1. Application for correction of military records (with supporting documents provided, if any).
2. Military Personnel Records and advisory opinions (if any).
THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show his social security number (SSN) as XXX-XX-XX62 vice XXX-XX-XX22.
2. The applicant states his DD Form 214 has an incorrect SSN.
3. The applicant provides his DD Form 214 and Social Security card.
CONSIDERATION OF EVIDENCE:
1. Title 10, U.S. Code, section 1552(b), provides that applications for correction of military records must be filed within 3 years after discovery of the alleged error or injustice. This provision of law also allows the Army Board for Correction of Military Records (ABCMR) to excuse an applicant's failure to timely file within the 3-year statute of limitations if the ABCMR determines it would be in the interest of justice to do so. While it appears the applicant did not file within the time frame provided in the statute of limitations, the ABCMR has elected to conduct a substantive review of this case and, only to the extent relief, if any, is granted, has determined it is in the interest of justice to excuse the applicant's failure to timely file. In all other respects, there are insufficient bases to waive the statute of limitations for timely filing.
2. The applicant enlisted in the U.S. Army Reserve (USAR) Delayed Entry Program (DEP) on 24 November 1982. His DD Form 4 (Enlistment/Reenlistment Document - Armed Forces of the United States) that was completed on that date shows his SSN with the last two digits as 62. He authenticated this form by placing his signature in the appropriate block on that date. He was discharged from the DEP on 14 April 1983.
3. He enlisted in the Regular Army (RA) on 15 April 1983. His DA Form 2-1 (Personnel Qualification Record) created upon his entry on active duty shows his SSN with the last two digits as 62.
4. His record contains numerous orders, personnel, medical, and legal documents that all show his SSN with the last two digits as 62. He authenticated many of these documents by placing his signature in the appropriate block.
5. He was honorably discharged from active duty on 12 January 1989. Item 3 (SSN) of his DD Form 214 shows his SSN with the last two digits as 22.
6. The applicant provides his Social Security card wherein it shows his SSN as shown on his DD Form 214 but with the last two digits as 62.
DISCUSSION AND CONCLUSIONS:
The evidence of record confirms the applicant listed his SSN with the last two digits as 62 upon his enlistment in the USAR and the RA. This SSN is consistent with the SSN he used throughout his period of military service. It appears an administrative error occurred when his DD Form 214 was prepared listing his SSN with the last two digits as 22. Therefore, it would be appropriate at this time to correct his DD Form 214 to show his SSN as shown on his Social Security card.
BOARD VOTE:

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by deleting from item 3 of his DD Form 214 the current entry and replacing it with the SSN as shown on his Social Security card.

IN THE CASE OF: BOARD DATE: 6 October 2015 DOCKET NUMBER: AR20150003819 THE BOARD CONSIDERED THE FOLLOWING EVIDENCE: 1. Application for correction of military records (with supporting documents provided, if any). The applicant's complete military records are not available to the Board for review.
ARMY | BCMR | CY2012 | 20120005584
The applicant requests correction of his DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) to show his social security number (SSN) as "XXX-XX-0030." Every document in his DA Form 201 (Military Personnel Records Jacket (MPRJ)) contains his Army-issued service number; the only document that contains his SSN is his DD Form 214. His DD Form 214 should therefore be corrected.
ARMY | BCMR | CY2011 | 20110023499
The applicant requests correction of Item 3 (Social Security Number (SSN)) on his DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge). His record contains: a. DD Form 398 (Statement of Personal History), dated 11 September 1972, that shows his SSN as "xxx-xx-7452 5274 7452"; b. DD Form 4 (Enlistment Record Armed Forces of the United States), dated 12 September 1973, that shows his SSN as "xxxxx5274 7452"; c. DA Form 41 (Record of Emergency Data), prepared on 19...

Item 3 (SSN) of the DD Form 214 he was issued shows his SSN with the last two digits as 35. DISCUSSION AND CONCLUSIONS: The evidence of record shows the applicant listed his SSN with the last two digits as 53 upon his induction into the AUS. Therefore, it would be appropriate at this time to correct his DD Form 214 to show his SSN as it is shown on his Medicare card.
ARMY | BCMR | CY2014 | 20140001982
The applicant states the SSN shown on his DD Form 214 is incorrect; the last two digits are reversed. His DD Form 47 (Record of Induction), dated 8 February 1968, shows his SSN with the last two digits as 43. It would be appropriate at this time to correct his DD Form 214 to show his SSN as it is shown on his DD Form 47.
ARMY | BCMR | CY2014 | 20140009904
The applicant requests correction of his DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) to show his social security number (SSN) as "XXX-XX-1547" instead of "XXX-XX-672." His DD Form 4 (Enlistment Record Armed Forces of the United States) identifies him by his service number; his SSN is not listed on this form. The evidence of record shows he enlisted in the Regular Army on 31 January 1966, and at the time of his enlistment his SSN was represented...

IN THE CASE OF: BOARD DATE: 2 April 2015 DOCKET NUMBER: AR20140015145 THE BOARD CONSIDERED THE FOLLOWING EVIDENCE: 1. The applicant states his SSN is incorrect on his DD Form 214; the last digit of his SSN is 5 and not 7. His DD Form 47 (Record of Induction) shows only his service number and not his SSN.
ARMY | BCMR | CY2005 | 20050001338C070206
The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show his correct social security number (SSN). The applicant provided a self-authored letter, dated 20 January 2005 in which he stated that he is requesting that his military and General Education Diploma be changed to show the correct SSN. Although the applicant's SSN is shown as XXX-XX-9933 on his social security card, he served on and was released from active duty in...
ARMY | BCMR | CY2012 | 20120004156
His correct SSN is xxx-xx-xx15 and not xxx-xx-xx51. His record contains a: a. DD Form 398 (Statement of Personal History), dated 8 September 1971, that does not list his SSN; b. DD Form 4 (Enlistment Record Armed Forces of the United States), dated 8 September 1971, that shows the last two digits of his SSN were changed from "51" to "15"; and c. Treasury Department Form SS-5 (Application for SSN (Or Replacement of Lost Card)), dated 8 September 1971, that shows he answered "yes" that he...
