THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 21 September 2012 to show his deployment to Afghanistan.

2.  The applicant states, in effect, his deployment to Afghanistan is not included on his DD Form 214.

3.  The applicant provides:

* deployment orders
* DD Form 214 for the period ending 21 September 2012

CONSIDERATION OF EVIDENCE:

1.  Having prior active service in the Regular Army (RA), the applicant again enlisted in the RA on 19 June 2000.  He served as a motor transport operator and medical logistics specialist and he was honorably discharged on 21 September 2012 for completion of required active service.

2.  His DD Form 214 for the period ending 21 September 2012 shows he was awarded or authorized the:

* Joint Service Commendation Medal
* Army Commendation Medal (3rd Award)
* Army Achievement Medal (3rd Award)
* Meritorious Unit Commendation
* Army Good Conduct Medal (3rd Award)
* National Defense Service Medal (2nd Award)
* Afghanistan Campaign Medal with two bronze service stars
* Global War on Terrorism Service Medal
* Korea Defense Service Medal
* Iraq Campaign Medal with one bronze service star
* Noncommissioned Officer Professional Development Ribbon
* Overseas Service Ribbon (4th Award)
* North Atlantic Treaty Organization Medal
* Driver and Mechanic Badge with Mechanic Bar

3.  Item 18 (Remarks) of this DD Form 214 shows the following deployments:

* Iraq from 11 February to 6 June 2003
* Iraq from 3 August 2007 to 20 June 2008
* Iraq from 19 January 2009 to 7 January 2010

4.  The Defense Finance and Accounting Service (DFAS) confirmed the applicant received hostile fire pay/imminent danger pay and combat zone tax exclusion for service in Afghanistan from 22 April 2011 through 22 February 2012.

5.  Army Regulation 600-8-22 (Military Awards) states a bronze service star is authorized for wear with the Iraq Campaign Medal for participation in each credited campaign.  Approved campaigns are:

* Liberation of Iraq (19 March 2003-1 May 2003)
* Transition of Iraq (2 May 2003-28 June 2004)
* Iraqi Governance (29 June 2004-15 December 2005)
* National Resolution (16 December 2005-9 January 2007)
* Iraqi Surge (10 January 2007-31 December 2008)
* Iraqi Sovereignty (1 January 2009-31 August 2010)
* New Dawn (1 September 2010-31 December 2011)

6.  Army Regulation 635-5 (Separation Documents), in effect at the time, prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army.  It established standardized policy for the preparation of the DD Form 214.  The regulation provided that for item 18 for an active duty Soldier deployed with his or her unit during their continuous period of active service, enter the statement "SERVICE IN (name of country deployed) FROM (inclusive dates, for example, YYYYMMDD-YYYYMMDD)."
DISCUSSION AND CONCLUSIONS:

1.  DFAS confirmed the applicant deployed to Afghanistan from 22 April 2011 through 22 February 2012.  Therefore, item 18 of his DD Form 214 for the period ending 21 September 2012 should be corrected to show his service in Afghanistan from 22 April 2011 to 22 February 2012.

2.  As a related issue, based on his service in Iraq and participation in four campaigns, he is authorized four bronze service stars with his already-awarded Iraq Campaign Medal.  Therefore, his DD Form 214 for the period ending 21 September 2012 should be corrected to show the appropriate number of service stars.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined the evidence presented is sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

	a.  deleting the Iraq Campaign Medal with one bronze service star from his DD Form 214 for the period ending 21 September 2012;

	b.  adding the Iraq Campaign Medal with four bronze service stars to his DD Form 214 for the period ending 21 September 2012; and



	c.  adding the entry "SERVICE IN AFGHANISTAN FROM 20110422-20120222" to item 18 of this DD Form 214 for the period ending 21 September 2012.
