THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests, in effect, correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show in:

* item 11 (Primary Specialty) -"92Y2O"
* item 18 (Remarks) - the correct name and street address of her nearest relative

2.  The applicant states, in effect, this information is incorrectly shown on her DD Form 214.

3.  The applicant provides her German marriage certificate, German registration, and Enlisted Record Brief (ERB).

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army on 29 July 2008.  She completed training and was awarded military occupational specialty 92Y (Unit Supply Specialist).

2.  On 24 March 2012, she was honorably released from active duty in the rank/grade of specialist (SPC)/E-4.  

3.  Her DD Form 214 shows in:

* item 11, the entry "92Y1O UNIT SUPPLY SPEC - 3 YRS 1 MOS
* Item 18, the entry "BLOCK 19b: XXXALG XXRAER;  2X AAxxNER STR; COLONGE NORD REIHN VESTFALIA [sic] 511XX; GERMANY"

4.  She provides her:

* German marriage certificate which shows spelling of her spouse's name as "XXXLIJ XXXKER
* German Registration which shows her address as "Acxxener Str. 2x" Postal Code 511xx Köln OT Porz
* ERB which shows her duty military occupational specialty (DMOS) as "92Y2O" in Section IX (Assignment Information) as of June 2011

5.  Army Regulation 635-5 (Separation Documents) at the time established the standardized policy for preparing and distributing the DD Form 214.  The regulation directs that the purpose of the separation document is to provide the individual with documentary evidence of his or her military service.  It is important that information entered on the form should be complete and accurate.  There is no provision to list an individual's DMOS on the DD Form 214.

6.  Köln (Cologne) is in the German Federal state of Nordrhein Westphalen (North Rhein-Westphalia).

DISCUSSION AND CONCLUSIONS:

1.  The evidence provided by the applicant is accepted as sufficient evidence to support correction of item 18 of the applicant's DD Form 214 to show the correct name and address of her nearest relative as listed on these documents.

2.  The record shows the applicant's PMOS at the time of her release from active duty was 92Y1O but that her DMOS was 92Y2O.  There is no provision to list a DMOS on the DD Form 214; therefore, there is no basis for granting this portion of the applicant's request.

BOARD DETERMINATION/RECOMMENDATION:

1.  The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by: 

	deleting from item 18 of her DD Form 214 the entry,"//BLOCK 19b: VXXALG MXXAER;  2X AAXXENER STR; COLONGE NORD REIHN VESTFALIA 511XX; GERMANY//" and replacing her spouse's name as shown on their German Marriage Certificate and street address as shown on the German Registration Form (the city/state to be Cologne, North Rhein-Westphalia).

2.  The Board further determined that the evidence presented was insufficient to warrant a portion of the requested relief.  As a result, the Board recommends denial of so much of the application that pertains to correcting the PMOS listed on her DD Form 214 to show the entry "92Y2O."
