BOARD DATE: 26 August 2014
DOCKET NUMBER: AR20140001511
THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:
1. Application for correction of military records (with supporting documents provided, if any).
2. Military Personnel Records and advisory opinions (if any).
THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) to show his social security number (SSN) as "X80-XX-XXXX" instead of "X08-XX-XXXX."
2. The applicant states the SSN shown on his DD Form 214 should be corrected to show "X80-XX-XXXX."
3. The applicant provides:
* social security card
* DD Form 214
CONSIDERATION OF EVIDENCE:
1. Title 10, U.S. Code, section 1552(b), provides that applications for correction of military records must be filed within 3 years after discovery of the alleged error or injustice. This provision of law also allows the Army Board for Correction of Military Records (ABCMR) to excuse an applicant's failure to timely file within the 3-year statute of limitations if the ABCMR determines it would be in the interest of justice to do so. While it appears the applicant did not file within the time frame provided in the statute of limitations, the ABCMR has elected to conduct a substantive review of this case and, only to the extent relief, if any, is granted, has determined it is in the interest of justice to excuse the applicant's failure to timely file. In all other respects, there are insufficient bases to waive the statute of limitations for timely filing.
2. The applicant was inducted into the Army of the United States on 30 August 1968. He served as a field artillery basic crewman in Vietnam from 6 May 1969 to 10 April 1970. He was released from active duty (REFRAD) on 10 April 1970.
3. His REFRAD orders, dated 10 April 1970, show his SSN as "X80-XX-XXXX."
4. Item 3 (SSN) of his DD Form 214 shows his SSN as "X08-XX-XXXX."
5. He provided a copy of his social security card that shows his SSN as
"X80-XX-XXXX."
DISCUSSION AND CONCLUSIONS:
The applicant's REFRAD orders show his SSN as "X80-XX-XXXX." Therefore, it would be appropriate to correct his DD Form 214 accordingly.
BOARD VOTE:
___X_____ ___X_____ __X___ GRANT FULL RELIEF
________ ________ ________ GRANT PARTIAL RELIEF
________ ________ ________ GRANT FORMAL HEARING
________ ________ ________ DENY APPLICATION
BOARD DETERMINATION/RECOMMENDATION:
The Board determined the evidence presented is sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by deleting the current entry in item 3 from his DD Form 214 and replacing it with the SSN as shown on his REFRAD orders.

The applicant requests, in effect, correction of the social security number (SSN) listed on his DD Form 214 (Report of Separation from Active Duty) for the period ending 30 November 1970. DISCUSSION AND CONCLUSIONS: The SSN shown on the applicant's social security card was recorded in his military records. In light of the available evidence, it would be appropriate to correct item 3 of both of his DD Forms 214 to show the SSN shown on his social security card.

This provision of law also allows the Army Board for Correction of Military Records (ABCMR) to excuse an applicant's failure to timely file within the 3-year statute of limitations if the ABCMR determines it would be in the interest of justice to do so. Item 3 (Social Security Number) of the DD Form 214 he was issued at the time shows his SSN as "xx9-xx-xxxx." He provides a copy of his Social Security Card, which shows his correct SSN, and it is different than the number shown on his DD...

BOARD DATE: 9 October 2014 DOCKET NUMBER: AR20140004373 THE BOARD CONSIDERED THE FOLLOWING EVIDENCE: 1. Application for correction of military records (with supporting documents provided, if any). His record is void of documentation listing the SSN he used during his period of active duty service; however, his record does contain an Army Reserve Personnel Data Review sheet signed by the applicant on 4 October 1966, approximately two months after his REFRAD, which lists his SSN as XX9-XX-XXXX.

IN THE CASE OF: BOARD DATE: 31 January 2012 DOCKET NUMBER: AR20110014974 THE BOARD CONSIDERED THE FOLLOWING EVIDENCE: 1. The applicant requests correction of his records to show his social security number (SSN) as 4x5-xx-xxxx instead of 5x4-xx-xxxx. 2. It appears an administrative error was made at the time his SSN was recorded on his DD Form 214.
ARMY | BCMR | CY2011 | 20110009836
The applicant requests correction of the social security number (SSN) shown on his DD Form 214 (Report of Transfer or Discharge). The evidence of record shows the SSN the applicant claims is correct was recorded upon his induction, but the first three digits were transposed on various documents in his record. Upon REFRAD, his SSN was incorrectly recorded on his DD Form 214 as "288-XX-XXXX."
ARMY | BCMR | CY2013 | 20130013623
The applicant requests correction of his DD Form 214 (Report of Separation from Active Duty) to show his social security number (SSN) as 231-XX-XXXX. DISCUSSION AND CONCLUSIONS: The evidence indicates an administrative error was made when recording the applicant's SSN on his DD Form 214. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending his DD Form 214 to show his SSN as shown on his social security card.
ARMY | BCMR | CY2012 | 20120021776
Application for correction of military records (with supporting documents provided, if any). The applicant states his DD Form 214 should list his SSN as "XX7-XX-XXXX" instead of "XX0-XX-XXXX." The DA Form 20 (Enlisted Qualification Record) prepared upon his entry on active duty lists his SSN as "XX7-XX-XXXX."
ARMY | BCMR | CY2012 | 20120022013
The applicant requests correction of his DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) to show his Social Security Number (SSN) as xx2-xx-xxxx instead of xx8-xx-xxxx. The applicant states the SSN shown on his DD Form 214 does not match the SSN on his Social Security card. The applicant's DD Form 4, DD Form 398, and DD Form 1584 list the SSN he claims is correct.
ARMY | BCMR | CY2013 | 20130012959
A DA Form 20 (Enlisted Qualification Record) with an audit date of 24 November 1969 shows his SSN entered as "587 XX XXXX." Army Regulation 635-5 (Separation Documents), effective 1 February 1967, stated the SSN in item 3 of the DD Form 214 was to be transcribed from the Soldier's DA Form 2139. DISCUSSION AND CONCLUSIONS: The evidence of record shows the SSN shown on the applicant's social security card was recorded on his DA Form 20.
