THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the Meritorious Service Medal. 

2.  The applicant states the award had not been received by the time her 
DD Form 214 was created. 

3.  The applicant provides a certificate (Meritorious Service Medal). 

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the U.S. Army Reserve on 11 February 2009.  She entered active duty on 17 March 2009 and she completed Officer Candidate School.  

2.  She was honorably discharged on 7 October 2009 to accept a commission. Her DD Form 214 for this period of service shows she was awarded or authorized the National Defense Service Medal.

3.  She was appointed as a Regular Army commissioned officer and executed an oath of office on 8 October 2009.  She completed the Quartermaster Officer Basic Course.  She served in Iraq from 3 July 2011 to 31 December 2011 and in Kuwait from 1 January 2012 to 27 June 2012.  

4.  She was honorably released from active duty on 1 June 2013.  Her DD Form 214 for this period of service shows she was awarded or authorized the:
* Army Commendation Medal
* National Defense Service Medal
* Global War on Terrorism Expeditionary Medal
* Global War on Terrorism Service Medal
* Iraq Campaign Medal with campaign star
* Army Service Ribbon
* Overseas Service Ribbon

5.  Permanent Orders (PO) Number 095-005, issued by Headquarters, 1st Cavalry Division, on 5 April 2013 awarded her the Meritorious Service Medal for service from March 2009 to June 2013. 

DISCUSSION AND CONCLUSIONS:

PO awarded her the Meritorious Service Medal which is not shown on her 
DD Form 214.  Therefore, she is entitled to correction of her DD Form 214 to show this award.

BOARD DETERMINATION/RECOMMENDATION:

As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding the Meritorious Service Medal to her DD Form 214 for the period ending 
1 June 2013.
