THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of the commencement date of her second period of initial active duty training service date to 11 December 2012.
2. The applicant states her U.S. Army Human Resources Command (AHRC) orders show a reporting date of 11 December 2012 not 13 February 2013 as shown on her DD Form 214 (Certificate of Release or Discharge from Active Duty).
3. The applicant provides copies of her 20 March 2013 DD Form 214, AHRC active duty orders, and Fort Sill, OK discharge orders.
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Oklahoma Army National Guard (ARNG) on 26 January 2012. She was ordered to initial active duty for training (IADT) on 3 April 2012.
2. The applicant was released from IADT on 17 August 2012. Her DD Form 214 shows 4 months and 15 days of active service with 2 months and 7 days of inactive service. Block 15 (Remarks) notes that she had not completed basic combat training.
3. AHRC Orders A-12-221773, dated 11 December 2012, ordered the applicant to active duty for the purpose of "Active Duty Medical Extension (ADME)." Her reporting date is shown as 11 December 2012 for a period not to exceed 179 days.
4. Her Enlisted Record Brief shows commencement of training on 19 December 2012.
5. On 20 March 2013, the applicant was released from active duty (AD) and discharged from the Reserve of the Army with a general discharge for misconduct (Drug Abuse).
6. The 20 March 2013 DD Form 214 provides the following at -
* block 12a (Date Entered AD This Period) - 13 February 2013
* block 12b (Separation Date This Period) - 20 March 2013
* block 12c (Net Active Service This Period) - 3 months, 10 days
* block 12d (Prior Active Service) - 4 months, 15 days
* block 12e (Prior Inactive Service) - 2 months, 7 days
7. Her National Guard Bureau (NGB) Form 22 (Report of Separation and Record of Service) shows service from 26 January 2012 through 20 March 2013, 1 year, 1 month, and 25 days. Her ARNG service was uncharacterized.
8. Other than the DD Form 214, the available record contains no documentation of any action on or about 13 February 2013.

2. The preponderance of evidence shows the applicant's second period of active duty commenced on 11 December 2012 not the 13 February 2013 as shown on her 20 March 2013 DD Form 214 and the duration of her period of service does not match the dates utilized. It is appropriate to correct this error.
3. The 20 March 2013 DD Form 214 does not include her total period of inactive in the ARNG service between her two periods of AD. It is appropriate to correct this error.
