IN THE CASE OF:
BOARD DATE: 10 July 2014
DOCKET NUMBER: AR20130017586
THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:
1. Application for correction of military records (with supporting documents provided, if any).
2. Military Personnel Records and advisory opinions (if any).
THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of her DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) to show her social security number (SSN) as X7X-74-XXXX.
2. The applicant states her SSN is X7X-74-XXXX, not X6X-78-XXXX as shown on her DD Form 214.
3. The applicant provides copies of her social security card and DD Form 214.
CONSIDERATION OF EVIDENCE:
1. Title 10, U.S. Code, section 1552(b), provides that applications for correction of military records must be filed within 3 years after discovery of the alleged error or injustice. This provision of law also allows the Army Board for Correction of Military Records (ABCMR) to excuse an applicant's failure to timely file within the 3-year statute of limitations if the ABCMR determines it would be in the interest of justice to do so. While it appears the applicant did not file within the time frame provided in the statute of limitations, the ABCMR has elected to conduct a substantive review of this case and, only to the extent relief, if any, is granted, has determined it is in the interest of justice to excuse the applicant's failure to timely file. In all other respects, there are insufficient bases to waive the statute of limitations for timely filing.
2. On 28 February 1966, the applicant enlisted in the Regular Army. Her records consistently show the use of a service number; however, the SSN
X7X-74-XXXX is also consistently used throughout her records.
3. On 28 November 1967, she was honorably discharged. Her DD Form 214 shows her SSN as X6X-78-XXXX.
4. The applicant's records contain a DA Form 41 (Record of Emergency Data) which shows her SSN as X7X-74-XXXX.
5. The applicant provided a copy of her social security card which shows her SSN as X7X-74-XXXX.
DISCUSSION AND CONCLUSIONS:
The evidence indicates an administrative error was made when recording the applicant's SSN on her DD Form 214. It would be appropriate to correct this error at this time.
BOARD VOTE:
___X____ ___X____ ___X____ GRANT FULL RELIEF
________ ________ ________ GRANT PARTIAL RELIEF
________ ________ ________ GRANT FORMAL HEARING
________ ________ ________ DENY APPLICATION
BOARD DETERMINATION/RECOMMENDATION:
The Board determined the evidence presented is sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by
amending her DD Form 214 to show her SSN as shown on her social security card.

Application for correction of military records (with supporting documents provided, if any). The FSM's records are void of and the applicant failed to provide evidence showing the requested SSN was ever used by the FSM during his military service. IN THE CASE OF: BOARD DATE: 20 December 2011 DOCKET NUMBER: AR20110012995 THE BOARD CONSIDERED THE FOLLOWING EVIDENCE: 1. It appears that during the preparation of the applicant's DD Form 214 a typographical error occurred resulting in his SSN being recorded as x7x-xx-xxxx instead of the SSN as shown on his Social Security card. Therefore, it would be at this time to correct the applicant's DD Form 214 to show the SSN as shown on his Social Security card.

The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show his social security number (SSN) as "x5x-6x-xxxx" instead of "x6x-3x-xxxx." The evidence he submitted also is not sufficient to show that an error exists or is a basis for...
