THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show in:
* item 11 (Primary Specialty) the number of years and months in his military occupational specialty (MOS)
* item 19a (Mailing Address after Separation) XX-XXX Wikao Street, Apartment XXXX, Mililani, HI 96789
2. The applicant states the mailing address after separation shown as
XX-XXX Wimao Street, Apartment XXXX, Kapolei, HI 96707 is incorrect and the number of years and months in specialty are shown as 00 years and 00 months.
3. The applicant provides his DD Form 214, separation orders, and a cable service invoice.
CONSIDERATION OF EVIDENCE:
1. The applicant's records show he enlisted in the Regular Army on 29 March 2006. He completed basic combat and advanced individual training and he was awarded MOS 92F (Petroleum Supply Specialist).
2. The exact date he was awarded MOS 92F is unknown. His Enlisted Record Brief is not available for review and his service records do not contain a certificate or a document confirming the exact date he completed the 10-week Petroleum Supply Specialist Course. His records do contain a Certificate of Training, dated 6 October 2006, showing he successfully completed the Additional Skill Identifier H7 Petroleum Vehicle Operator Course at Fort Leonard Wood, MO, on or about that date.
3. He served through multiple extensions and/or reenlistments and he attained the rank/grade of specialist/E-4. His last unit of assignment was Company A, 325th Brigade Support Battalion, 3rd Brigade, 25th Infantry Division, Schofield Barracks, HI.
4. He was discharged under honorable conditions (general) on 17 July 2013 under the provisions of Army Regulation 635-200 (Active Duty Enlisted Administrative Separations) chapter 14. His DD Form 214 shows in:
* item 11 92F1O PETROLEUM SUPPLY SP 00 YRS 00 MOS
* item 19a XX-XXX WIMAO ST. APT XXXX, KAPOLEI, HAWAII 96707
5. The applicant provided a cable service invoice, dated 16 August 2013, showing the requested mailing address.
6. The Board analyst of record attempted to contact the applicant on 14 April 2014 in an effort to obtain his MOS orders or a verification of MOS completion date. The applicant did not respond.
7. Army Regulation 635-5 (Separation Documents), in effect at the time, established the standardized policy for preparing and distributing the DD Form 214. The purpose of the separation document is to provide the individual with documentary evidence of his or her military service. It is important that information entered on the form should be complete and accurate. Chapter 2 contained specific guidance for preparation of the DD Form 214.
a. For item 11, enter the titles of all MOS's awarded and held for at least 1 year during the current period of service and include the number of years and months held for each MOS. For time determinations, 16 or more days count as 1 month. Do not count time in training (basic training, advanced individual training, or MOS-producing school).
b. For item 19a, the mailing address after separation will be provided by the Soldier. This address must be a permanent address. Civilian penal institutions will not be accepted. Enter the street address, city, state, and zip code.
DISCUSSION AND CONCLUSIONS:
1. The entry in item 11 pertaining to the number of years and months in his MOS is clearly in error; however, there is insufficient evidence to correct this entry. The applicant's records are void of orders awarding him MOS 92F or a certificate of training/completion confirming the date he completed training for this MOS. Estimates of dates of completion and/or duration of courses are speculative and do not serve the interest of justice. Therefore, there is insufficient evidence to correct this entry.
2. The entry in item 19a pertaining to the mailing address after separation was provided by the applicant. He also digitally authenticated the DD Form 214 by placing his signature in the proper block. Absent convincing independent and verifiable evidence to the contrary, it must be presumed that the applicant's mailing address was correct at the time and there is an insufficient evidentiary basis for changing it. Therefore, he is not entitled to the requested relief.
