THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his address after separation and the address of his nearest relative on his DD Form 214 (Certificate of Release or Discharge from Active Duty).

2.  The applicant states the correct addresses are:

	a.  Item 19a (Mailing Address after Separation):  2--- 2--- Ave, P----- City, Alabama  36867; and 

	b.  Item 19b (Nearest Relative (Name and Address)):  4--- K-------- Blvd, 
D------- Springs, Florida  32433. 

3.  The applicant provides copies of his DD Form 214 and DD Form 215 (Correction to DD Form 214)

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army on 8 October 2008.  He held military occupational specialty 11C (Indirect Fire Infantryman).

2.  He was retired due to temporary disability on 23 July 2013.  He had completed 4 years, 9 months, and 16 days of active duty service.  His DD Form 214 shows: 

	a.   mailing address after separation as 1--- O------ Lakes, Blvd, D-------, Florida 32433; and

	b.  his nearest relative address:  2--- P----- City, Alabama 35867.

3.  Army Regulation 635-5 (Separation Documents) prescribes the separation documents that must be prepared for Soldiers on retirement, discharge, or release from active service or control of the Active Army.  It establishes standardized policy for preparing and distributing the DD Form 214.  It states for   item 19 (Mailing Address after Separation and Nearest Relative) will be provided by the soldier.

		(1)  This address must be a permanent address. 

		(2)  The name and address of a relative should be someone who will know the Soldier's location and address at all times.

DISCUSSION AND CONCLUSIONS:

1.  The applicant requests the addresses shown in items 19a and 19b of his DD Form 214 be corrected.  In view of the importance of correct information in these items it would be appropriate to make the requested corrections.

BOARD DETERMINATION/RECOMMENDATION:

1.  The Board determined the evidence presented is sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

	a.  deleting the current entry in item 19a of his DD Form 214 and replacing it with the address he shows on his application; and 

	b.  deleting the address shown in item 19b and replacing it with the address shown on his application.
