THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:
1. Application for correction of military records (with supporting documents provided, if any).
2. Military Personnel Records and advisory opinions (if any).
THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his records to show his deployment to Iraq.
2. The applicant states he was deployed to Iraq from September 2008 through September 2009 but his DD Form 214 does not show this fact.
3. The applicant provides no supporting documentation.
CONSIDERATION OF EVIDENCE:
1. The applicant, with prior service, enlisted in Regular Army on 20 November 2005. He served in the military occupational specialties 88M (Motor Transport Operator) and 88K (Watercraft Operator).
2. The applicant was awarded two Army Commendation Medals.
a. The first, on 30 August 2009, for exceptional service while serving as a team leader in support of combat operations during Operation Iraqi Freedom with the 89th Transportation Company, 260th Combat Sustainment Support Battalion, 10th Sustainment Brigade from 15 September 2008 to 31 August 2009.
b. The second, on 25 June 2010, for meritorious service while assigned to the 89th Transportation Company, 6th Transportation Battalion, 7th Sustainment Brigade from 16 October 2007 to 30 June 2010.
3. On 19 March 2013, he was discharged with severance pay by reason of a physical disability.
4. His DD Form 214 (Certificate of Release or Discharge from Active Duty) shows no foreign service or entry in the Remarks block indicating a deployment. It lists his awards as the Army Commendation Medal, Army Achievement Medal (2nd award), Army Good Conduct Medal, National Defense Service Medal, Global War On Terrorism Service Medal, and Army Service Ribbon.
5. A review of the applicant's pay records shows he received hazardous duty/ imminent danger pay for a deployment to Kuwait/Iraq from 16 September 2008 through 13 September 2009 (11 months, 27 days).
6. Army Regulation 635-5 (Separation Documents) prescribes the separation documents that must be prepared for Soldiers on retirement, discharge, release from active duty service, or control of the Active Army. It establishes standardized policy for preparing and distributing the DD Form 214 and states for an active duty Soldier deployed with their unit during their continuous period of active service, enter the following statement "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD - YYYYMMDD)."
7. Army Regulation 600-8-22 (Military Awards) states:
a. The Iraq Campaign Medal is awarded to members who have served in direct support of Operation Iraqi Freedom. The area of eligibility encompasses all the land area of the country of Iraq, the contiguous water area out to 12 nautical miles, and all air spaces above the land area of Iraq and above the contiguous water area out to 12 nautical miles. The Iraq Campaign Medal period of eligibility is on or after 19 March 2003 through 31 December 2011. A bronze service star is authorized for wear with this medal for participation in each credited campaign. Approved campaigns are:
* Liberation of Iraq (19 March 2003-1 May 2003)
* Transition of Iraq (2 May 2003-28 June 2004)
* Iraqi Governance (29 June 2004-15 December 2005)
* National Resolution (16 December 2005-9 January 2007)
* Iraqi Surge (10 January 2007-31 December 2008)
* Iraqi Sovereignty (1 January 2009-31 August 2010)
* New Dawn (1 September 2010-31 December 2011)
b. The Overseas Service Ribbon is awarded for successful completion of overseas tours.
DISCUSSION AND CONCLUSIONS:
1. The applicant served in Kuwait/Iraq for just shy of a year; however, except for his Army Commendation Medal citation there is no indication of this service in his official records.
2. Based on his period of service in Iraq, the applicant is authorized receipt of the Iraq Campaign Medal with two bronze service stars.
3. Neither his period of foreign service nor the mandatory deployment entry were included on his DD Form 214. It is appropriate to correct his DD Form 214 to include these entries.
4. The applicant is also authorized receipt of an Overseas Service Ribbon based on his deployment.
5. Additionally, the applicant was awarded two Army Commendation Medals; his DD Form 214 does not reflect this fact. It is appropriate to correct the DD Form 214 to show the second award.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:
a. deleting the entry of the Army Commendation Medal from his DD Form 214; and
b. adding the following to his DD Form 214 at:
(1) block 12f (Foreign Service) - 0000 11 27;
(2) block 13 (Decorations, Medals, Badges, Citations And Campaign Ribbons Awarded Or Authorized) - the Army Commendation Medal (second award), Iraq Campaign Medal with two bronze service stars, and the Overseas Service Ribbon; and
(3) block 18 (Remarks), - "SERVICE IN IRAQ/KUWAIT FROM 20080916 - 20090913."
