THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the amount of severance pay she received as $11,887.20.

2.  The applicant states her DD Form 214 incorrectly reflects that she received $15,849.00 in severance pay benefits when, in fact, she received $11,887.20 in severance pay.

3.  The applicant provides a copy of her DD Form 214 and her DA Form 5892 (Physical Evaluation Board Liaison Officer (PEBLO) Estimated Disability Compensation Worksheet).

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army on 1 July 2009 for a period of 3 years and 29 weeks and training as a health care specialist.  She completed her training; served overseas in Korea; and was transferred to Fort Hood, Texas.

2.  On 20 December 2012, she was honorably discharged under the provisions of Army Regulation 635-40 (Physical Evaluation for Retention, Retirement, or Separation), chapter 4, due to disability with severance pay, non-combat (enhanced).  She completed 3 years, 5 months, and 20 days of active service.  Her DD Form 214 shows she received $15,849.60 in severance pay benefits.

3.  The DA Form 5892 provided by the applicant shows her severance pay was calculated by the PEBLO as being $11,887.20.

4.  Information received from the Defense Finance and Accounting Service confirms the applicant received $11,887.20 in severance pay benefits.

DISCUSSION AND CONCLUSIONS:

1.  The applicant's contention that her DD Form 214 reflects the incorrect amount of severance pay she received was noted and found to have merit.

2.  He DD Form 214 reflects that she received $15,849.50 in severance pay benefits when, in fact, she only received $11,887.20.  Accordingly, her DD Form 214 should be corrected to show this amount.

The Board determined the evidence presented is sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

* deleting the entry "Disability Severance Pay -- $15,849.60" from her DD Form 214
* adding the entry "Disability Severance Pay -- $11,887.20" to her DD Form 214
