THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty):

	a.  Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized):  Add the Defense Meritorious Service Medal; and 

	b.  item 19b (Nearest Relative): show his wife's name as Raeshawn A____, not Raeshaen S. A____.

2.  The applicant states his wife’s first name is misspelled.

3.  The applicant provides his:

* DD Form 214
* Identification and Privilege Card
* Retirement Data Sheet
* Retirement orders
* Defense Meritorious Service Medal approval memorandum, citation, and award certificate

CONSIDERATION OF EVIDENCE:

1.  The applicant is a Regular Army master sergeant.  He is scheduled to retire on 30 June 2013.

2.  He was issued a DD Form 214 when he departed on transition leave.  His 
DD Form 214 does not show the Defense Meritorious Service Medal. 

3.  He provides:

	a.  An approved packet indicating he was awarded the Defense Meritorious Service Medal on 10 April 2013, for service from September 2009 to June 2013. 

	b.  A Retirement Data Sheet on which he indicated his wife's name as Raeshawn A____.

DISCUSSION AND CONCLUSIONS:

1.  The applicant was awarded the Defense Meritorious Service Medal which is not shown on his DD Form 214.  He is entitled to correction of his DD Form 214 to show this award. 

2.  The applicant provided a Retirement Data Sheet showing his wife's name.  His DD Form 214 should be corrected.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

	a.  adding the Defense Meritorious Service Medal to item 13 of his DD Form 214; and 

	



b.  showing his wife's name in item 19b of his DD Form 214 as Raeshawn A____.
