THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests reconsideration of his previous request for correction of his DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge), item 3 (Social Security Number (SSN)).
2. He states he was not aware his SSN was incorrect on his DD Form 214 until he attempted to obtain healthcare benefits from the Department of Veterans Affairs. He did not use his SSN during his period of service and when he was discharged he did not verify the information listed. He was more concerned with returning home to his loved ones. The error is now affecting benefits.
3. He provides his DD Form 214 and a Social Security Administration SSN Printout.
CONSIDERATION OF EVIDENCE:
1. Incorporated herein by reference are military records which were summarized in the previous consideration of the applicant's case by the Army Board for Correction of Military Records (ABCMR) in Docket Number AR20120002880 on 9 August 2012.
2. The applicant has provided new evidence that requires consideration at this time.
3. The applicant's complete military service record is not available for review. This case is being considered using a partial record provided by the National Personnel Records Center (NPRC) and the documents the applicant provides.
4. A DD Form 47 (Record of Induction) shows the applicant was inducted into the Army of the United States on 24 September 1965. The form lists a service number, which was the primary identification number used in military records at the time. The form does not list an SSN.
5. His DA Form 20 (Enlisted Qualification Record) does not show his SSN.
6. His record contains a punch card dated 1 September 1967 bearing his name and SSN in the following format: 2XXX XX XXX.
7. On 22 September 1967, he was honorably released from active duty. The DD Form 214 he was issued at that time shows his SSN as "3XX XX XXXX."
8. The available records do not include a DA Form 2139 (Military Pay Voucher).
9. His record at NPRC is filed under SSN 2XX XX XXXX.
10. He provides a Social Security Administration SSN Printout showing his SSN is 2XX-XX-XXXX.
11. Army Regulation 635-5 (Separation Documents) prescribes the separation documents that must be prepared for Soldiers on retirement, discharge, or release from active duty service or control of the Active Army. It establishes standardized policy for preparing and distributing the DD Form 214. The regulation in effect at the time of the applicant's separation stated the SSN entered on the DD Form 214 would be transcribed from DA Form 2139.
DISCUSSION AND CONCLUSIONS:
1. Although the available military records are largely void of references to the applicant's SSN, these records do show the SSN he claims is correct was entered on a punch card prior to his release from active duty. This is also the SSN under which his military records are filed at the NPRC.
2. The available evidence indicates an administrative error resulted in the entry of an incorrect SSN on his DD Form 214. It would be appropriate correct item 3 of his DD Form 214 to show his SSN as it is shown on the Social Security Administration SSN Printout he provides.

The Board determined that the evidence presented was sufficient to warrant amendment of the ABCMRs decision in Docket Number AR20120002880, dated 9 August 2012. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by replacing the entry in item 3 of his DD Form 214 with the SSN shown on the Social Security Administration SSN Printout.
