THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests, in effect, correction of an entry in item 18 (Remarks) on his DD Form 214 (Certificate of Release or Discharge from Active Duty). 

2.  The applicant states he "did not receive a $45,000 sign on bonus, it was only $15,000."

3.  The applicant provides:

* enlistment contract
* DD Form 214

CONSIDERATION OF EVIDENCE:

1.  The applicant's enlistment contract shows he enlisted in the Regular Army on 1 April 2009 for an enlistment bonus in the amount of $15,000.  

2.  He served as an infantryman in Afghanistan from 12 April 2011 to 6 January 2012.  On 8 April 2013, he was retired by reason of permanent disability.    

3.  Item 18 of his DD Form 214 shows, among other entries, the entry "ENLISTMENT BONUS PAID:  $45,000.00, 20110331."    




4.  Records from the Defense Finance and Accounting Service (DFAS) confirm he received an enlistment bonus in the amount of $15,000.  

5.  Army Regulation 635-5 (Personnel Separations - Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army.  It establishes standardized policy for the preparation of the DD Form 214.  It states for a Soldier who received an enlistment/reenlistment bonus enter the type of bonus, amount, and when issued in item 18.

DISCUSSION AND CONCLUSIONS:

The applicant's DD Form 214 shows he received an enlistment bonus in the amount of $45,000 on 31 March 2011.  However, his enlistment contract and DFAS records show he received an enlistment bonus in the amount of $15,000.  Therefore, it would be appropriate to correct item 18 of his DD Form 214 to show the amount of his enlistment bonus was $15,000.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by deleting the entry "ENLISTMENT BONUS PAID:  $45,000.00, 20110331" from item 18 of his DD Form 214 and replacing it with the entry "ENLISTMENT BONUS PAID:  $15,000.00, 20110331."
