THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Meritorious Service Medal with 2nd Oak Leaf Cluster.
2. The applicant states his retirement award was not included on his DD Form 214.
3. The applicant provides:
* documents showing he was awarded the Meritorious Service Medal for the period 30 January 2002 to 31 January 2012
* Meritorious Service Medal Certificate with permanent order number showing he was awarded the Meritorious Service Medal for the period 31 August 1997 to 31 July 2001
CONSIDERATION OF EVIDENCE:
1. The applicant is a retired U.S. Army Reserve sergeant first class/E-7.
2. His DD Form 214 does not show award of the Meritorious Service Medal.
3. He provides documents showing he was awarded:
* the Meritorious Service Medal on 2 June 2001 for exceptionally meritorious service from 31 August 1997 to 31 July 2001
* the Meritorious Service Medal with 1st Oak Leaf Cluster on 13 August 2012 for exceptionally meritorious service from 30 January 2002 to 31 January 2012
4. His Enlisted Record Brief, dated 10 August 2009, shows the Meritorious Service Medal listed among his awards and decorations.
DISCUSSION AND CONCLUSIONS:
1. The applicant has provided evidence showing he has been awarded two Meritorious Service Medals. Neither is listed on his DD Form 214. His DD Form 214 should be corrected to show these awards.
2. He requested correction of his DD Form 214 to show he was awarded the Meritorious Service Medal with 2nd Oak Leaf Cluster, indicating a total of three Meritorious Service Medals. There is no evidence he was awarded three Meritorious Service Medals.

BOARD DETERMINATION/RECOMMENDATION:
1. The Board determined the evidence presented is sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding the Meritorious Service Medal (2nd Award) to his DD Form 214.
2. The Board further determined the evidence presented is insufficient to warrant a portion of the requested relief. As a result, the Board recommends denial of so
much of the application that pertains to correcting his DD Form 214 to show he was awarded the Meritorious Service Medal with 2nd Oak Leaf Cluster.
