THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show his home of record as Ohio.
2. The applicant states he was never a resident of Oregon; he was visiting Oregon when he enlisted. He was born, raised, and currently resides in Mansfield, Ohio, which should be his home of record.
3. The applicant provides copies of his DD Form 214, high school transcript, and birth certificate.
CONSIDERATION OF EVIDENCE:
1. The applicant was born on 10 May 1989. On 20 February 2008, he enlisted in the U.S. Army Reserve (USAR) Delayed Entry Program for 8 years which provided for his enlistment in the Regular Army not later than 18 June 2008. During the enlistment process, he completed and signed a DD Form 4 (Enlistment/Reenlistment Document Armed Forces of the United States) that listed his home of record as Noti, Oregon. This is the only home of record shown in his military records.
2. The applicant also completed a Security Clearance Application, dated 20 February 2008. This form shows he lived from 19 January 2008 to the present in Noti, OR, and he lived in Mansfield, OH, from 19 February 1990 to 18 January 2008. He listed three references of persons who knew him well, all living in Ohio. His father lived in Mansfield, Ohio; his mother lived in Oregon.
3. The applicant enlisted in the Regular Army and served on active duty from 18 June 2008 until 18 February 2012 when he was honorably released and transferred to the USAR in pay grade E-4. The home of record shown on his DD Form 214 is Noti, Oregon.
4. Army Regulation 601-280 (Total Army Retention Program) provides instructions for completing the DD Form 4 series. For Home of Record, it states to enter the address the Soldier claims as a permanent address. It defines the term Home of Record as the place recorded as the home of the individual when commissioned, appointed, enlisted, inducted, or ordered into the relevant tour of active duty.
DISCUSSION AND CONCLUSIONS:
1. The home of record shown on the applicant's DD Form 214 is the one he provided when he enlisted. It is the only home of record listed in his military records.
2. However, his Security Clearance Application lends credence to his contention that he was only visiting Oregon when he enlisted. The form indicates he lived in Oregon for only one month prior to his enlistment; he lived in Ohio the previous 18 years; all persons who knew him well lived in Ohio; his mother lived in Oregon but his father lived in Mansfield, OH. His contention that his home of record was Mansfield, OH, and he was only visiting Oregon when he enlisted is accepted..

BOARD DETERMINATION/RECOMMENDATION:
