THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests two additional awards of the Army Achievement Medal (AAM) be added to his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 29 December 2011.

2.  The applicant states at the time of his discharge he did not have the orders with him and they were not in the interactive Personnel Electronic Records Management System (iPERMS). 

3.  The applicant provides:

* Orders for three awards of the AAM
* DD Form 214 for the period ending 29 December 2011

CONSIDERATION OF EVIDENCE:

1.  The applicant was ordered to active duty from the U.S. Army Reserve on 
13 February 2011 in support of Operation Enduring Freedom.  On 29 December 2011, he was released from active duty.

2.  Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) of his DD Form 214 for the period ending 29 December 2011 shows one award of the AAM.

3.  Permanent Orders #136-01, dated 16 May 2002, show he received the AAM for meritorious service during the period 17 December 1999 to 16 July 2002. 
4.  Permanent Orders #336-006, undated, show he received the AAM for meritorious achievement during the period 25 October 2005 to 30 September 2006. 

5.  Permanent Orders #317-01, dated 12 November 2008, show he received the AAM for meritorious achievement during the period 8 May 2008 to 14 November 2008. 

DISCUSSION AND CONCLUSIONS:

The applicant's DD Form 214 for the period ending 29 December 2011 shows one award of the AAM but orders show he received three awards of the AAM.  Therefore, this DD Form 214 should be corrected to show two additional awards of the AAM.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by deleting the entry "ARMY ACHIEVEMENT MEDAL" from item 13 of his DD Form 214 for the period ending 29 December 2011 and replacing it with the entry "Army Achievement Medal (3rd Award)."
