THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests that his Social Security Number (SSN) be corrected.
2. The applicant states he just recently realized that the SSN on his DD Form 214 (Report of Transfer or Discharge) is incorrect.
3. The applicant provides a copy of a severely-worn Social Security Card with the last four digits of "1449" and his DD Form 214
CONSIDERATION OF EVIDENCE:
1. Title 10, U.S. Code, section 1552(b), provides that applications for correction of military records must be filed within 3 years after discovery of the alleged error or injustice. This provision of law also allows the Army Board for Correction of Military Records (ABCMR) to excuse an applicants failure to timely file within the 3-year statute of limitations if the ABCMR determines it would be in the interest of justice to do so. While it appears the applicant did not file within the time frame provided in the statute of limitations, the ABCMR has elected to conduct a substantive review of this case and, only to the extent relief, if any, is granted, has determined it is in the interest of justice to excuse the applicants failure to timely file. In all other respects, there are insufficient bases to waive the statute of limitations for timely filing.
2. During the period of this applicants service the principle identification number utilized was the Service Number (SN).
3. The applicant served on active duty from 18 February 1964 through 17 February 1966.
4. His DD Form 214, block 32 (Remarks), shows an SSN (shown as a Social Security Account Number (SSAN)) with the last four digits as "1259."
5. The applicant's record contains only four documents containing the SSN -- three show the SSN with the last four digits as "1259" and one document lists the last four digits of his SSN as "1449."
6. The Social Security Card provided appears to be a well-worn version of the cards issued in the 1960's. It shows an SSN listing the last four digits as "1449."
7. Army Regulation 635-5 (Separation Documents) prescribes the separation documents that must be prepared for Soldiers on retirement, discharge, release from active duty service, or control of the Active Army. It establishes standardized policy for preparing and distributing the DD Form 214 and states personnel are to ensure all information on the DD Form 214 and other separation documents are accurate. The DD Form 214 is of vital importance to the separating Soldier and must be properly prepared according to prescribed guidance. No deviation is authorized.
DISCUSSION AND CONCLUSIONS:
1. The available records show two different SSN's were utilized during the period the applicant served in addition to a Service Number. Given that his record contains the SSN listed on his DD Form 214 and the SSN listed on his Social Security Card it reasonable to presume that the correct SSN is the one listed on his Social Security Card.
2. Therefore, it would be appropriate to correct the DD Form 214 to show the SSN as it is listed on his Social Security Card.

As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by deleting the SSN that is listed on the applicant's DD Form 214 in block 32 and adding the SSN as it is listed on his Social Security Card.
