THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his social security number (SSN) on "deployment orders" and correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show his deployment to participate in Operation Iris Gold.

2.  He states upon retiring and picking up his DD Form 214, he realized his participation in Operation Iris Gold is not listed.  After finding his "deployment orders," he realized his SSN was wrong and that the last four numbers of the SSN of the Soldier listed below him were entered as the last four numbers of his SSN.   

3.  He provides:

* a memorandum
* his DD Form 214 with a DD Form 214C (Continuation Sheet)
* an Army Achievement Medal (AAM) Certificate
* a DA Form 638-1 (Recommendation for Award (for Other than Valor) of AAM, Army Commendation Medal, and Meritorious Service Medal)
* a Noncommissioned Officer Evaluation Report 

CONSIDERATION OF EVIDENCE:

1.  On 16 July 1987, the applicant enlisted in the Regular Army.  After completing initial entry training, he was awarded military occupational specialty (MOS) 94B (Food Service Specialist), and he later served in MOS 92G (Food Service Operations).

2.  His record shows he served the entirety of his military service using 
SSN xxx-xx-2010.

3.  His record includes an AAM Certificate showing he was awarded the AAM for meritorious achievement during Operation Iris Gold from 7 to 12 December 1991.

4.  Item 5 (Oversea Service) of his DA Form 2-1 (Personnel Qualification Record) shows he was deployed to Kuwait from 7 through 12 December 1991. 

5.  He retired on 30 April 2012 after completing 24 years, 9 months, and 15 days of net active service.  Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) of his DD Form 214 shows, in part, he was authorized the Southwest Asia Service Medal (SWASM) with one bronze service star.  Item 18 (Remarks) does not include an entry documenting his deployment to Kuwait in 1991.

6.  His record does not contain orders pertaining to his deployment to Kuwait in 1991.

7.  He provides a memorandum, subject: Southwest “Asian” Service Medal [with] Bronze Star, dated 18 December 1991.  The memorandum confirmed the applicant and several other Rangers were deployed in support of Operation Iris Gold from 6 to 12 December 1991 and were authorized the SWASM with one bronze service star.  Distribution instructions on the memorandum state one copy was to be distributed to each individual concerned.  

8.  His SSN is listed on the memorandum as "xxx-xx-8867."  

9.  Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army.  It states for an active duty Soldier deployed with his or her unit during his or her continuous period of active service, in item 18 enter the statement:  "SERVICE IN (name of country deployed) FROM (inclusive dates for example, YYYYMMDD-YYYYMMDD)."

DISCUSSION AND CONCLUSIONS:

1.  The evidence of record shows the applicant deployed to Kuwait from 7 to 12 December 1991.  It would be appropriate to add a statement documenting this service to item 18 of his DD Form 214.  
2.  It appears the memorandum he has provided is the "deployment orders" he references in his application.  Although his SSN is incorrect on this document, the error does him no harm.  The memorandum is not part of the official record in his Army Military Human Resource Record (AMHRR) (formerly known as the Official Military Personnel File), and it appears it was not distributed further than the individuals concerned.  Further, his service during Operation Iris Gold is documented on an AAM Certificate in his record.  He should also note that these Proceedings will be filed in his AMHRR and may be used to verify his deployment for Operation Iris Gold.

BOARD DETERMINATION/RECOMMENDATION:

1.  The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding to item 18 of his DD Form 214 the statement " SERVICE IN KUWAIT FROM 19911207-19911212."

2.  The Board further determined that the evidence presented was insufficient to warrant a portion of the requested relief.  As a result, the Board recommends denial of so much of the application that pertains to correction of his SSN.
