THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests reconsideration of his earlier request for correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the Combat Infantryman Badge, Southwest Asia Service Medal, and Kuwait Liberation Medal.
2. The applicant states the requested awards are not listed on his DD Form 214.
3. The applicant provides Permanent Orders 26-21 awarding him the Combat Infantryman Badge.
CONSIDERATION OF EVIDENCE:
1. Incorporated herein by reference are military records which were summarized in the previous consideration of the applicant's case by the Army Board for Correction of Military Records (ABCMR) in Docket Number AC97-10017, on 19 November 1997.
2. The applicant provides permanent orders awarding him the Combat Infantryman Badge which were not previously considered. Therefore, it is new evidence and as such warrants consideration by the Board.
3. The applicant enlisted in the Regular Army on 1 November 1988 and he held military occupational specialty 11M (Fighting Vehicle Infantryman). He served in Saudi Arabia from 27 December 1990 to 13 May 1991.
4. He was honorably released from active duty on 6 February 1992. His DD Form 214 shows the:
* Army Service Ribbon
* National Defense Service Medal
* Army Lapel Button
* Good Conduct Medal
* Sharpshooter Marksmanship Qualification Badge with Rifle Bar (M-16)
* Expert Marksmanship Qualification Badge with Grenade Bar
5. On 18 February 1993, he was issued a DD Form 215 (Correction to DD Form 214) that added his service in Southwest Asia as well as awards of the:
* Southwest Asia Service with three bronze service stars
* Kuwait Liberation Medal
6. On 27 July 2011, he was issued a second DD Form 215 that added the Valorous Unit Award.
7. He provides Permanent Orders 26-21, issued by Headquarters, 1st Infantry Division, on 18 April 1991, awarding him the Combat Infantryman Badge.
8. Army Regulation 600-8-22 (Military Awards) provides, in pertinent part, that the Kuwait Liberation Medal awarded by the Kingdom of Saudi Arabia (KLM-SA) was approved on 3 January 1992 and is awarded to members of the Armed Forces of the United States who participated in the Persian Gulf War between 17 January 1991 and 28 February 1991. Additionally, the Kuwait Liberation Medal awarded by the Government of Kuwait (KLM-K) was approved on 9 November 1995 and is awarded to members of the Armed Forces of the United States who participated in the Persian Gulf War between 2 August 1990 and 31 August 1993.
DISCUSSION AND CONCLUSIONS:
1. Permanent orders awarded him the Combat Infantryman Badge which is not shown on his DD Form 214. Therefore, he is entitled to correction of his DD Form 214 to show it.
2. The evidence of record shows that the applicant served in Southwest Asia from 27 December 1990 to 13 May 1991; therefore, he served a qualifying period for award of the Kuwait Liberation Medal awarded by the Kingdom of Saudi Arabia and the Kuwait Liberation Medal awarded by the Government of Kuwait and is entitled to correction of his DD Form 214 to show these awards.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant amendment of the ABCMRs decision in Docket Number AC97-10017, dated 19 November 1997. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:
* voiding all previously-issued DD Forms 214 and 215
* issuing him a new DD Form 214 that lists his service in Southwest Asia as shown on the DD Form 215 dated 18 February 1993 as well as award of the:
* Army Service Ribbon
* National Defense Service Medal
