THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests item 15a (Commissioned Through Service Academy) of his DD Form 214 (Certificate of Release or Discharge from Active Duty) be changed to reflect "YES" instead of "NO."
2. The applicant states the wrong box is checked in item 15a of his
DD Form 214. He graduated and received his commission from the U.S. Military Academy at West Point, NY, on 26 May 1982.
3. The applicant provides his:
* DD Form 214
* U.S. Military Academy Academic Transcript, prepared on 14 June 1983
* Officer Record Brief (ORB), dated 20 May 2011
CONSIDERATION OF EVIDENCE:
1. He was commissioned as a second lieutenant on 26 May 1982 at West Point, NY.
2. The U.S. Military Academy Academic Transcript he submitted shows he graduated on 26 May 1982 with a Bachelor of Science degree.
3. His ORB shows his basic date of appointment as 26 May 1982. The source of his original appointment is shown as the U.S. Military Academy.
4. He was retired on 31 May 2010 and he was placed on the Retired List as a lieutenant colonel the following day. Line 15a of his DD Form 214 has an X in the "NO" block.
DISCUSSION AND CONCLUSIONS:
His transcripts and ORB are sufficient to show an error occurred when
item 15a of his DD Form 214 was completed. Therefore, it is appropriate to correct item 15a by deleting the X in the "NO" block and entering an X in the "YES" block.
