THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests reconsideration of her previous application for correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show her foreign service in support of Operations Desert Shield/Desert Storm and awards authorized for that service.

2.  She states she was awarded the appropriate service medals, but her DD Form 214 was not corrected.  She served her country by going to war in Saudi Arabia and she wants her records to reflect her foreign service.

3.  She provides copies of:

* DD Form 214
* Army Board for Correction of Military Records (ABCMR) Docket Number AC95-09921, dated 10 April 1996

CONSIDERATION OF EVIDENCE:

1.  Incorporated herein by reference are military records which were summarized in the previous consideration of the applicant's case by the ABCMR in Docket Number AC95-09921 on 10 April 1996.

2.  She provides a new argument which will be considered by the Board in her case.

3.  She enlisted in the Regular Army on 30 August 1988.  After completion of training, she served in military occupational specialty 94B (Food Service).

4.  The Gulf War Participants Roster shows the applicant was deployed in support of the Gulf War for the period 13 February through 16 March 1991.

5.  On 21 February 1992, she was honorably released from active duty.  She completed 3 years, 5 months, and 22 days of net active service and 2 years and 4 days of foreign service.  Her DD Form 214 does not show her deployment.  It shows she was awarded the:

* Army Service Ribbon
* Army Good Conduct Medal (1st Award)
* National Defense Service Medal
* Overseas Service Ribbon

6.  The previous Record of Proceedings states the applicant's service finance records indicated she deployed to Southwest Asia on 12 December 1990 in support of Operations Desert Shield/Desert Storm while assigned to a unit in Germany.  She remained in Southwest Asia until 10 March 1991.

7.  Her record is void of a DD Form 215 (Correction to DD Form 214); however, it does contain a letter from the U.S. Army Reserve Personnel Center, dated 5 December 1996.  This document shows a DD Form 215 and a DA Form 1577 (Authorization for Issuance of Awards) were mailed to the applicant's address in Nashville, TN.

8.  The DA Form 1577, dated 2 December 1996, shows she was awarded the Southwest Asia Service Medal (SWASM) with two bronze service stars and the Kuwait Liberation Medal (KLM) awarded by the Kingdom of Saudi Arabia 
(KLM-SA).

9.  Army Regulation 600-8-22 (Military Awards) states the KLM awarded by the Government of Kuwait (KLM-KU) was approved on 9 November 1995 and is awarded to members of the Armed Forces of the United States who participated in the Persian Gulf War between 2 August 1990 and 31 August 1993.

10.  Army Regulation 635-5 (Separation Documents), in effect at the time, did not provide for the entry of the location or duration of deployments on the DD Form 214.

11.  Army Regulation 635-5, currently in effect, states the DD Form 214 of an active duty Soldier deployed with his or her unit during their continuous period of active service will have the statement "SERVICE IN (name of country deployed) FROM (inclusive dates for example, YYYYMMDD-YYYYMMDD)" entered in item 18 (Remarks).

DISCUSSION AND CONCLUSIONS:

1.  In ABCMR Docket Number AC95-09921, the Board confirmed the applicant deployed in support of Operations Desert Shield/Desert Storm from 12 December 1990 through 10 March 1991.  As a result, item 18 of her DD Form 214 should be corrected to add this deployment.

2.  The evidence shows she served in Saudi Arabia during a period for which the KLM-KU was awarded.  As such, she is also entitled to correction of her DD Form 214 to show this award.

3.  She contends that although she was issued the appropriate awards for her service during Operations Desert Shield/Desert Storm, her DD Form 214 was not corrected.

4.  The applicant's record is void of a DD Form 215.  It appears this document, assuming it was completed, was not filed in her record or provided to her.  As such, it would be equitable to issue her a DD Form 215 that also includes the SWASM and KLM-SA.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined the evidence presented is sufficient to warrant amendment of the ABCMR's decision in Docket Number AC95-09921, dated 10 April 1996.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by issuing her a DD Form 215 which amends the following items of her DD Form 214:

	a.  item 13 (Decorations, Medals, Badges, Citations, and Campaign Ribbons Awarded or Authorized) by adding the following awards:

* SWASM
* KLM-SA
* KLM-KU

	b.  item 18 by adding the entry "SERVICE IN SAUDI ARABIA FROM 19901212-19910310."
