THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:
1. Application for correction of military records (with supporting documents provided, if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his:
* Bronze Star Medal Certificate to show his last name as W---e- vice W---o- on line six of the citation portion of the certificate
* Purple Heart Certificate to show the date he was wounded in action in Vietnam as 13 January 1969 vice 16 January 1969
2. The applicant states both certificates were issued on 16 December 2010 and contain the abovementioned errors.
3. The applicant provides two certificates.
CONSIDERATION OF EVIDENCE:
1. The applicant was inducted into the Army of the United States on 26 February 1968 and he held military occupational specialty 11B (Light Weapons Infantryman). He served in Vietnam from 25 July 1968 through 21 January 1969.
2. General Orders Number 8, issued by Headquarters, 312th Evacuation Hospital, Vietnam, dated 16 January 1969, awarded him the Purple Heart for wounds received in action in Vietnam on 13 January 1969.
3. General Orders Number 1633, issued by Headquarters, Americal Division, Vietnam, dated 11 March 1969, awarded him the Bronze Star Medal with "V" Device for heroism in connection with military operations against a hostile force in Vietnam on 13 January 1969. It correctly reflects the applicant's name as
W---e- throughout these orders.
4. The applicant was honorably released from active duty on 29 September 1969 and he was transferred to the U.S. Army Reserve (USAR) Control Group (Annual Training) to complete his remaining Reserve obligation. He completed 1 year, 7 months, and 4 days of total active service.
5. Item 24 (Decorations, Medals, Badges, Commendations, Citations and Campaign Ribbons Awarded or Authorized) of the DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) he was issued at the time, as corrected by a DD Form 215 (Correction to DD Form 214), dated 4 October 1989, shows he was awarded the Purple Heart and the Bronze Star Medal with "V" Device.
6. The applicant provides a Bronze Star Medal Certificate, dated 16 December 2010, wherein his last name is shown as "Wxxxor" vice "Wxxxer" on line six of the citation portion of the certificate.
7. He also provides a Purple Heart Certificate, dated 16 December 2010, wherein it shows he received the Purple Heart for wounds received in action on "16 January 1969" vice "13 January 1969."
8. Army Regulation 600-8-22 (Military Awards) states the bronze V device indicates acts of heroism involving conflict with an armed enemy and authorizes the device in conjunction with several awards, including the Bronze Star Medal.
DISCUSSION AND CONCLUSIONS:
1. The applicant's Bronze Star Medal Certificate incorrectly shows his last name W---o- vice W---e- on line six of the citation portion of his certificate. Therefore, his Bronze Star Medal Certificate should be corrected to show the correct spelling of his last name.
2. The applicant's Purple Heart Certificate incorrectly shows he was wounded in action in Vietnam on 16 January 1969 vice 13 January 1969. General orders show he was wounded in action on 13 January 1969. It appears a typographical error was made when this certificate was prepared. Therefore, his Purple Heart Certificate should be corrected to show he was wounded in action in Vietnam on 13 January 1969.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:
a. issuing him a new Bronze Star Medal Certificate showing his last name in the certificate as shown in the "TO" name line of his Bronze Star Medal Certificate
b. issuing him a new Purple Heart Certificate showing the date he was wounded in action in Vietnam as 13 January 1969.
