THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of item 12b (Separation Date), item 12c (Net Service Completed This Period), and item 14 (Military Education) of his DD Form 214 (Certificate of Release or Discharge from Active Duty).

2.  The applicant states his DD Form 214 erroneously lists his separation date as 19 October 2010 in item 12b when, in fact, he was not separated until 16 December 2010.  He also states item 14 should document his attendance and completion of the Biological Integration Detection Systems (JBPDS) course from 30 November 2010 through 16 December 2010.

3.  The applicant provides the following documents in support of his application:

* DD Form 214
* basic combat training orders, dated 19 May 2010
* military occupational specialty (MOS) orders, dated 3 December 2010
* U.S. Army Maneuver Support Center and Fort Leonard Wood, Missouri, memorandum, dated 14 December 2010
* DA Form 1059 (Service School Academic Evaluation Report (AER)), dated 15 December 2010

CONSIDERATION OF EVIDENCE:

1.  The applicant's record shows he enlisted in the U.S. Army Reserve for 8 years on 10 November 2009 and he entered active duty for training on 25 May 2010.

2.  The applicant's DD Form 214 shows he was released from active duty for training on 19 October 2010 in item 12b and he completed 4 months and 26 days of active duty service during the period covered by the DD Form 214 in item 12c.  Item 14 does not include the JBPDS course.

3.  The applicant's record contains an AER, dated 16 December 2010, showing the applicant attended the JBPDS course from 30 November 2010 through 16 December 2010 and he achieved course standards.

4.  The record also contains a memorandum from the U.S. Army Maneuver Support Center and Fort Leonard Wood, to The Adjutant General of Utah, dated 14 December 2010, requesting correction of the applicant's DD Form 214.  It requests correction of items 12b and 12c to show his separation date as 16 December 2010 and completion of 6 months and 21 days of active duty service during the period covered by the DD Form 214, respectively.

DISCUSSION AND CONCLUSIONS:

1.  The applicant's request for correction of items 12b, 12c, and 14 of his DD Form 214 has been carefully considered and found to have merit.

2.  The evidence of record confirms the applicant was actually released from active duty on 16 December 2010, as opposed to 19 October 2010, as indicated in item 12b of his DD Form 214.  It further shows he completed 6 months and 21 days of active duty service from the date of his entry on active duty on 25 May 2010 through the date of his separation on 16 December 2010, as opposed to 4 months and 25 days as indicated in item 12c of his DD Form 214.  In addition, as evidenced by the AER on file, the applicant achieved course standards and completed the JBPDS course during this period of active duty service.  As a result, it would be appropriate to correct items 12b, 12c, and 14 of his DD Form 214 accordingly.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined the evidence presented is sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending his DD Form 214 as follows:

	a.  item 12b – delete the current entry and replace it with the entry "2010  12  16;

	b.  item 12c – delete the current entry and replace it with the entry "0000  06  21"; and

	c.  item 14 – add the entry "Biological Integration Detection Systems (JBPDS), 2 weeks, 2010."
