THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that his 7 January 2003 DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show his service in Afghanistan.

2.  The applicant states he served in Afghanistan during Operation Enduring Freedom from 7 July through 1 December 2002.  The period of foreign service is included in item 12f (Foreign Service) on the DD Form 214, but the country of service is not noted in item 18 (Remarks).

3.  The applicant provides copies of a DA Form 638 (Recommendation for Award), his 7 January 2003 and 17 August 2011 DD Forms 214, and his DA Form 2-1 (Personnel Qualification Record).

CONSIDERATION OF EVIDENCE:

1.  The applicant, as a Regular Army enlisted Soldier, served on active duty from 8 July 1999 through 7 January 2003.

2.  The applicant was awarded the Army Commendation Medal for the period 7 July 2002 through 6 January 2003.  The achievements noted include a reference to his deployment to Afghanistan during Operation Enduring Freedom.

3.  Item 12f of his 7 January 2003 DD Form 214 shows 4 months and 23 days of foreign service.  Item 18 does not contain a statement related to the country in which the applicant's foreign service was performed.
4.  His DA Form 2-1 does not provide any clarifying information as to his deployment to Afghanistan.

5.  A review of the applicant's pay records by the Defense Finance and Accounting Service (DFAS) shows the applicant was in receipt of hazardous duty/imminent danger pay for Afghanistan from 11 July 2002 through 1 December 2002.

6.  Army Regulation 635-5 (Separation Documents) prescribes the separation documents that must be prepared for Soldiers on retirement, discharge, release from active duty service, or control of the Active Army.  It establishes standardized policy for preparing and distributing the DD Form 214 and provides that:

	a.  the total amount of foreign service completed during the period covered by the DD Form 214 is to be entered at item 12f; and

	b.  item 18, for an active duty Soldier deployed with his or her unit, will contain the statement "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example YYYYMMDD - YYYYMMDD)."

DISCUSSION AND CONCLUSIONS:

The evidence of record clearly shows the applicant served in Afghanistan from 11 July 2002 through 1 December 2002.  It would be appropriate to correct the applicant's 7 January 2003 DD Form 214, item 18, to reflect this service.

As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding in item 18 of his 7 January 2003 DD Form 214 the statement "SERVICE IN AFGHANISTAN FROM 20020711 - 20021201."
