THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the Army Achievement Medal (4th Award).
2. The applicant states he received the award after he was separated from active duty.
3. The applicant provides his DD Form 214, orders, and a certificate.
CONSIDERATION OF EVIDENCE:
1. The applicant's records show he was ordered to active duty as a member of his Army National Guard (ARNG) unit in support of Operation Enduring Freedom on 11 January 2010.
2. He was honorably released from active duty on 27 December 2010 and assigned to the ARNG. He completed 11 months and 17 days of creditable active service during this period of service.
3. The DD Form 214 he was issued for this period of service shows, in pertinent part, the Army Achievement Medal (3rd Award).
4. His record contains orders for award of the Army Achievement Medal as follows:
* Permanent Orders (PO) Number 09-196-003, dated 22 August 2009, issued by Task Force Griffin, Iraq, for the period 21 November 2008 to 21 August 2009
* PO Number 175-5, dated 24 June 2010, issued by 2nd Battalion, 356th Logistic Support Regiment, Fort Bliss, TX, for the period 1 December 2009 to 12 June 2010
* PO Number 349-01, dated 8 December 2010, issued by 2nd Battalion, 356th Logistic Support Regiment, Fort Bliss, TX, for the period 11 January 2010 to 10 January 2011
5. The applicant provides PO Number 349-01 as shown above.
DISCUSSION AND CONCLUSIONS:
The evidence of record shows the applicant was awarded three awards of the Army Achievement Medal which is correctly shown on his DD Form 214. Therefore, he is not entitled to the requested relief.
