		BOARD DATE:	  11 October 2011

		DOCKET NUMBER:  AR20110008289

THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1. Application for correction of military records (with supporting documents provided, if any).

2. Military Personnel Records and advisory opinions (if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1. The applicant requests correction of the DD Form 214 (Report of Separation from the Armed Forces of the United States) of her deceased husband, a former service member (FSM), to show his correct date of birth (DOB).

2. The applicant states the FSM’s correct DOB is 20 October 1930, not
20 December 1930.

3. The applicant provides the FSM's:

* DD Form 214
* Birth Certificate from the State of Minnesota
* Death Certificate from the State of Minnesota

CONSIDERATION OF EVIDENCE:

1. The FSM's military records are not available to the Board for review. A fire destroyed approximately 18 million service members’ records at the National Personnel Records Center in 1973. It is believed that the FSM's records were lost or destroyed in that fire. However, there were sufficient documents remaining in a reconstructed record for the Board to conduct a fair and impartial review of this case.

2. The applicant's DD Form 214 (Report of Separation from the Armed Forces of the United States) shows he was:

* born on 20 October 1930
* inducted into the Army of the United States on 27 February 1952
* separated on 19 February 1954

3. The applicant submitted a copy of the FSM’s birth certificate and death certificate that shows his DOB as 20 October 1930.

DISCUSSION AND CONCLUSIONS:

The available evidence is sufficient to reasonably establish that the FSM's DOB is 20 October 1930. Therefore, the FSM's DD Form 214 should be corrected to show his DOB as 20 October 1930.

BOARD VOTE:

___x_____  ____x____  ___x_____  GRANT FULL RELIEF

________  ________  ________  GRANT PARTIAL RELIEF

________  ________  ________  GRANT FORMAL HEARING

________  ________  ________  DENY APPLICATION

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by deleting the entry in item 10 (DOB) of his DD Form 214 and adding 20 October 1930.

The FSM's complete military records are not available for review. There is no evidence of record available and the applicant has provided insufficient evidence to show her...
