THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 2 January 2011 to shows award of the:

* National Defense Service Medal (NDSM) (3rd Award)
* Marksman Marksmanship Qualification Badge with Rifle and Grenade Bars
* Sharpshooter Marksmanship Qualification Badge with Machine Gun 
(M-249) and Pistol Bars
* Network+ Course
* Basic Information Management Operators Course

2.  He states, in effect, that his DD Form 214 incorrectly shows two awards of the NDSM instead of three.  The above marksmanship badges were omitted from his DD Form 214.  He continues that the two-education course should be added to his DD Form 214.

 3.  He provides three DD Forms 214, for the periods ending 29 September 2005, 29 September 2008, and 2 January 2011; a DA Form 7304-R (Scorecard for M249 AR), dated 31 January 2010, a DA Form 3595-R (Record Fire Scorecard), dated 5 February 2010; a Network+ Certificate, dated 27 August 2010; and a  Basic Information Management Operators Course Certificate, dated 1 September 2010.


CONSIDERATION OF EVIDENCE:

1.  The applicant had prior active duty service in the United States Navy (USN).

2.  He enlisted in the Regular Army on 30 September 2005.  On 29 September 2008, he was honorably separated from active duty and he was transferred to the United States Army Reserve.

3.  Item 13 (Decorations, Medals, Badges, Citations, and Campaign Ribbons Awarded or Authorized) shows award:

* Army Commendation Medal
* Army Achievement Medal
* Navy Unit Commendation
* Navy Meritorious Unit Commendation (3rd Award)
* USN “E” Ribbon
* Navy Good Conduct Medal (3rd Award)
* NDSM
* Army Forces Expeditionary Medal
* Global War on Terrorism Expeditionary Medal
* Global War on Terrorism Service Medal
* Armed Forces Service Medal
* Iraq Campaign Medal with one bronze service star
* Army Service Ribbon
* Navy Sea Service Deployment Ribbon (2nd Award)
* USN Submarine Insignia
* Sharpshooter Marksmanship Qualification Badge with Pistol Bar
* Marksman Marksmanship Qualification Badge with Rifle Bar
* Marksman Marksmanship Qualification Badge with Grenade Bar

4.  On 14 January 2010, he was ordered to active duty in support of Operation Iraqi Freedom.  

5.  He submitted a DA Form 3595-R, dated 31 January 2010, which shows he received a score of 40 and qualified as sharpshooter during weapons firing for the (M-249) machine gun.  There are no orders in his service records that show he was awarded the Sharpshooter Marksmanship Qualification Badge with Machine Gun Bar.

6.  He submitted a Certificate of Achievement, dated 27 August 2010, that shows he completed the Network+ Course, at Camp Arifjan, Kuwait during the period 23 August 2010 through 27 August 2010.

7.  He submitted a Certificate of Training, dated 27 August 2010, that shows he completed the Basic information Management Operators Course, at Camp Arifjan, Kuwait during the period 31 August 2010 through 1 September 2010.

8.  On 2 January 2011, he was honorably released from active duty.

9.  Item 13 of the applicant’s DD Form 214 shows the NDSM (2nd Award), it does not show award of the Marksman Marksmanship Qualification Badge with Rifle and Grenade Bars nor does it show award the Sharpshooter Marksmanship Qualification Badge with Pistol and Machine Gun Bars.  Item 14 (Military Education) does not show he completed the Network+ Course or the Basic information Management Operators Course.

10.  Army Regulation (AR) 600-8-22 (Military Awards) provides that the NDSM is awarded for honorable active service for any period between 27 July 1950 through 27 July 1954, 1 January 1961 through 14 August 1974, 2 August 1990 through 30 November 1995, and 11 September 2001 to a date to be determined. Second and subsequent awards of the NDSM are denoted by a bronze service star affixed to the NDSM. 

11.  AR 635-5 (Separation Documents), in effect at that time, established the policies and procedures for completion and distribution of the DD Form 214.  In pertinent part, it states for item 13 list awards and decorations for all periods of service in priority sequenced.  Entries will be verified by the Soldier’s records.  Item 14 states list in-service training course one week or longer by; title, number of weeks, year successfully completed during this period of service; e.g., medical, dental, electronics, supply, administration, personnel, or heavy equipment operations.  This information is to assist the member after separation in job placement and counseling; therefore, training courses for combat skills will not be listed.

DISCUSSION AND CONCLUSIONS:

1.  The applicant contends that his DD Form 214 should show award of the NDSM (3rd Award).  Evidence of record shows he entered active duty on 
20 December 1994 and he was released from active duty on 29 September 2008.  He transferred to the USAR 30 September 2008 and he serves in the USAR up to the present.  In accordance with AR 600-8-22 he is eligible for award of two awards of the NDSM from his active duty entry date to the present.  Therefore, his DD Form 214 for the period ending 2 January 2011 is correct.

2.  Evidence of record shows he was awarded the Sharpshooter Marksmanship Qualification Badge with Machine Gun Bar.  His DD Form 214 should be amended to add the Sharpshooter Marksmanship Qualification Badge with Machine Gun Bar.  

3.  The applicant’s DD Form 214 for the period ending 29 September 2008 shows he was awarded the Sharpshooter Marksmanship Qualification Badge with Pistol Bar and the Marksman Marksmanship Qualification Badge with Rifle and Grenade Bars.  His DD Form 214 for the period ending 2 January
2011 should be amended to add award of the Sharpshooter Marksmanship Qualification Badge with Pistol Bar and the Marksman Marksmanship Qualification Badge with Rifle and Grenade Bars

4.  Evidence of record shows that the applicant completed Network+ Course, at Camp Arifjan, Kuwait.  Therefore, his DD Form 214 should be corrected to add Network+ Course, 1 week, (2010) in item 14.

5.  The applicant's Basic Information Management Operators Course was less than one week in length, making it not eligible for entry on his DD Form 214.

BOARD DETERMINATION/RECOMMENDATION:

1.  The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

	a.  amending his DD Form 214 in item 13 to add:

* Sharpshooter Marksmanship Qualification Badge with Machine Gun Bar
* Sharpshooter Marksmanship Qualification Badge with Pistol Bar
* Marksman Marksmanship Qualification Badge with Rifle Bar
* Marksman Marksmanship Qualification Badge with Grenade Bar; and 

	b.  amending his DD Form 214 in item 14 to add the Network+ Course, 
1 week, (2010).

2.  The Board further determined that the evidence presented is insufficient to warrant a portion of the requested relief.  As a result, the Board recommends denial of so much of the application that pertains to changing the NDSM to show third award and by adding the Basic Information Management Operators Course to his DD Form 214.
