THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:
1. Application for correction of military records (with supporting documents provided, if any).
2. Military Personnel Records and advisory opinions (if any).
THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his records to show his foreign service for participation in two operations, "Operation Desert Fox" during the period
18 August 1998 through 15 May 1999 and "Operation Enduring Freedom" during the period 10 March 2003 through 15 November 2003.
2. The applicant states that because of the high volume of workload of S1 personnel many mistakes were made and many Soldiers did not receive recognition for their military operations.
3. The applicant provides his DA Form 2166-8 (NCO Evaluation Report (NCOER)) and DD Form 214 (Certificate of Release or Discharge from Active Duty).
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Regular Army on 4 September 1996. He completed basic combat and advanced individual training and was awarded military occupational specialty (MOS) 19K (Armor Crewman) and later was awarded MOS 91Q (Pharmacy Specialist). The highest rank/grade he attained while serving on active duty was sergeant/E-5.
2. The applicant was honorably released from active duty on 31 October 2003 after completing 7 years, 1 month, and 27 days of active service.
3. The applicant's DD Form 214 does not show any deployments. Item 12f (Foreign Service) does not show he served any foreign service. His DD Form 214 shows he was awarded the Army Achievement Medal (2d Award), the Army Good Conduct Medal, the National Defense Service Medal, the Noncommissioned Officer Professional Development Ribbon, and the Army Service Ribbon.
4. During the processing of this case, the Defense Finance and Accounting Service (DFAS) reviewed his Master Military Pay Account (MMPA). His MMPA shows he received hostile fire pay/imminent danger pay (HFP/IDP) and the combat zone tax exclusion for Kuwait from 19 August 1998 through
26 December 1998 and from 21 March 2003 through 18 June 2003 (7 months and 6 days of foreign service).
5. During the processing of this case, a member of the Board's staff reviewed the unit's history of 115th Field Hospital, Ft. Polk, Louisiana, the unit to which the applicant was assigned. Documents show that the 115th Field Hospital deployed to Kuwait in 2003 in support of "Operation Iraqi Freedom."
6. The applicant provided his NCO Evaluation Report (NCOER) which shows he displayed technical proficiency in his duties as a pharmaceutical technician during an Operation Enduring Freedom deployment.
7. Army Regulation 635-5 (Personnel Separations - Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214. It states that for an active duty Soldier deployed with his or her unit during their continuous period of active service, enter the statement "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD-YYYMMDD)" in block 18 of the DD Form 214.
8. A review of the applicant's records indicates entitlement to an additional award and decoration that is not shown on his DD Form 214.
9. Army Regulation 600-8-22 (Military Awards) states the Global War on Terrorism Expeditionary Medal (GWOTEM) is authorized for award to members of the Armed Forces of the United States who deployed abroad for service in the Global War on Terrorism operations (to include service in Kuwait) on or after 11 September 2001 to a date to be determined. The general area of eligibility
(AOE) encompasses all foreign land, water, and air spaces outside the fifty states of the United States and outside 200 nautical miles of the shores of the United States in operations approved by the Secretary of Defense.
DISCUSSION AND CONCLUSIONS:
1. DFAS records indicate the applicant was deployed to Kuwait.
2. The 115th Field Hospital unit's history document confirms that the organization was deployed in support of "Operation Iraqi Freedom" in 2003.
3. The unit's history documents and DFAS information combined with the applicant's statement and NCOER are accepted as sufficient evidence to amend block 18 of the applicant's DD Form 214 to show he served two tours of duty in Kuwait during the period 19 August 1998 through 26 December 1998 and during the period 21 March 2003 through 18 June 2003 and to show in item 12f that he served 7 months and 6 days of foreign service.
4. Based on his service in Kuwait in 2003, it would also be appropriate to add the Global War on Terrorism Expeditionary Medal to his DD Form 214.

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:
a. adding the statement "SERVICE IN KUWAIT FROM "19 August 1998 through 26 December 1998 and 21 March 2003 through 18 June 2003" to item 18 of his DD Form 214.
b. adding 7 months and 6 days to item 12f of his DD Form 214, and
c. adding the Global War on Terrorism Expeditionary Medal to his DD Form 214.
