THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:
1. Application for correction of military records (with supporting documents provided, if any).
2. Military Personnel Records and advisory opinions (if any).
THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests his DD Form 214 (Certificate of Release or Discharge from Active Duty), Items 7a (Place of Entry into Active Duty) and 7b (Home of Record (HOR) At Time of Entry), be changed to "Massachusetts."
2. The applicant states he entered active duty in Massachusetts and he lives in Massachusetts.
3. The applicant provides his DD Form 214 and a letter from the Town Clerk of Warren, MA which verifies the applicant has been a registered voter in the town of Warren since 1984.
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Army National Guard (ARNG) on 22 October 2002 with prior enlisted service in the Marine Corps and Marine Corps Reserve. He also had prior ARNG service as an officer. Throughout all of his service his address and HOR was Warren, MA.
2. He served on active duty from 17 December 2007 to 12 June 2008 in support of Operation Enduring Freedom. The DD Form 214 he was issued for this service shows in Item 7a "Warren, Maryland" and in Item 7b "1XX Bragg Rd, Warren Maryland 01083."
DISCUSSION AND CONCLUSIONS:
1. It is abundantly clear that the applicant has always lived in Massachusetts and Maryland was entered on his DD Form 214 in error.
2. In view of the foregoing, it would be appropriate to correct the applicant's records as recommended below.

a. Item 7a "WARREN, MASSACHUSETTS"; and
b. Item 7b "1XX BRAGG RD, WARREN MASSACHUSETTS 01083."
