THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Forms 214 (Certificate of Release or Discharge from Active Duty) for the period ending 16 October 2005 and
23 March 2009 to show award of the Army Good Conduct Medal (AGCM) (1st Award) instead of the U.S. Coast Guard (USCG) Good Conduct Medal. He also requests correction of his DD Form 214 for the period ending 23 March 2009 to show award of the Bronze Star Medal (BSM) with V Device.
2. The applicant states his DD Forms 214 incorrectly show he was awarded the USCG Good Conduct Medal. He also states his last DD Form 214 does not show the BSM with V Device.
3. The applicant provides:
* his DD Forms 214 for the period ending 16 October 2005 and 23 March 2009
* his DA Form 638 (Recommendation for Award) showing his award of the BSM with V Device and BSM Certificate
* Permanent Orders 183-042 dated 2 July 2005 for award of the AGCM
CONSIDERATION OF EVIDENCE:
1. The applicant was born on 11 August 1980 and he enlisted in the New York Army National Guard (NYARNG) on 31 December 1997 for a period of 8 years. He completed his one-station unit training (OSUT) at Fort Benning, GA and was returned to his NYARNG unit.
2. On 7 April 2004, he reenlisted for a period of 3 years and on 15 May 2004 he was ordered to active duty in support of Operation Iraqi Freedom (OIF). He deployed to Iraq on 6 October 2004 and on 1 January 2005 he was promoted to the rank/grade of sergeant (SGT)/E-5. He departed Iraq on 9 September 2005.
3. Records show Permanent Order 183-042, issued by Company B,
1st Battalion, 69th Infantry, 256th Infantry Brigade, dated 2 July 2005, awarded the applicant the first award of the AGCM. His record also contains the associated AGCM Certificate.
4. On 16 October 2005, he was honorably released from active duty (REFRAD). Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) of his DD Form 214 for this period of service shows among other awards the USCG Good Conduct Medal.
5. On 17 January 2008, he was ordered to active duty in support of Operation Enduring Freedom (OEF) and he deployed to Afghanistan from 29 March 2008 to
3 January 2009.
6. On 20 February 2009, the applicant was awarded the BSM with V Device for exceptional valor on 22 October 2008 by Permanent Order Number 051-004, issued by Headquarters, Combined Security Transition Command - Afghanistan, Camp Eggers, Kabul, Afghanistan.
7. On 23 March 2009, he was honorably REFRAD at Fort Bragg, NC. Item 13 of his DD Form 214 for this period shows also shows the "Coast Guard Good Conduct Medal." However, there is no entry for the BSM with V Device on this DD Form 214.
DISCUSSION AND CONCLUSIONS:
1. The applicant never served in the USCG and he was awarded the AGCM. However, through an administrative error, the applicants DD Forms 214 for the period ending 16 October 2005 and 23 March 2009 incorrectly show he was awarded the USCG Good Conduct Medal. Accordingly, these DD Forms 214 should be corrected to show the Army Good Conduct Medal (1st Award).
2. The applicant was also awarded the BSM with V Device. Therefore, his
DD Form 214 for the period ending 23 March 2009 should be corrected to show this award.

BOARD DETERMINATION/RECOMMENDATION:
1. The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:
a. deleting from item 13 of his DD Forms 214 for the period ending
16 October 2005 and 23 March 2009 the entry "U.S. Coast Guard Good Conduct Medal" and replacing them with the entry "Army Good Conduct Medal (1st Award)"; and
b. adding to item 13 of his DD Form 214 for the period ending 23 March 2009 the Bronze Star Medal with "V" Device.
2. The Board wants the applicant and all others to know that the sacrifices he makes in service to the United States during the Global War on Terrorism are deeply appreciated. The applicant and all Americans should be justifiably proud of his service in arms.
