THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests the entry "DD Form 215 will be issued to provide missing information" be removed from his DD Form 214 (Certificate of Release or Discharge from Active Duty) with a separation date of 18 May 2009.

2.  The applicant states he received a letter, dated 2 September 2010, from the National Personnel Records Center (NPRC), St. Louis, MO, stating there is no DD Form 215 (Correction to DD Form 214) in his electronic record.

3.  The applicant provides:

* a letter from NPRC, dated 2 September 2010
* his DD Form 214

CONSIDERATION OF EVIDENCE:

1.  He enlisted in the Regular Army on 16 August 2007 for a period of 2 years and 2 weeks.  He completed basic combat and advanced individual training and was awarded military occupational specialty 11B (Infantryman).

2.  On 18 May 2009, he was discharged by reason of misconduct (serious offense).  He completed 1 year, 8 months, and 28 days of active service.

3.  None of the blocks on his DD Form 214 contain the entry "See Block 18."  Block 18 (Remarks) contains the entry "DD Form 215 will be issued to provide missing information."

4.  His official military personnel file (OMPF) in the integrated Personnel Electronic Records Management System does not show a DD Form 215 was issued.

5.  Army Regulation 635-5 (Separation Documents) governs the preparation of the DD Form 214 and states if required information is missing when completing a DD Form 214 enter "See Block 18" in the incomplete block.  In block 18, enter "DD Form 215 will be issued to provide missing information."

DISCUSSION AND CONCLUSIONS:

1.  The blocks on his DD Form 214 all contain valid entries and none of the blocks contain the entry " See Block 18."  Therefore, it is reasonable to conclude there was no missing information at the time his DD Form 214 was completed.

2.  His OMPF does not show a DD Form 215 has been issued.

3.  In view of the above, it appears the entry "DD Form 215 will be issued to provide missing information" was entered on his DD Form 215 in error.  Therefore, it is appropriate to remove this entry from his DD Form 214.

The Board determined the evidence presented is sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by 

deleting the entry "DD Form 215 will be issued to provide missing information" from block 18 of his DD Form 214 with a separation date of 18 May 2009.
