THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of item 7a (Place of Entry Into Active Duty) of his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 2 August 2010 to show "Kankakee, Illinois" instead of "Kankakee, Wisconsin."

2.  The applicant states:

* It is an error
* He was with the 317th Engineer Company in Kankakee, IL

3.  The applicant provides a copy of his DD Form 214 for the period ending 
2 August 2010.

CONSIDERATION OF EVIDENCE:

1.  The applicant was ordered to active duty from the U.S. Army Reserve on       3 October 2009 in support of Operation Iraqi Freedom (OIF).  He served as a construction equipment operator in Kuwait/Iraq from 16 November 2009 to        17 July 2010.  On 2 August 2010, he was released from active duty by reason of completion of required active service.

2.  Item 7a of his DD Form 214 for the period ending 2 August 2010 shows the entry "KANKAKEE, WISCONSIN."
3.  Orders 09-209-00029, issued by Headquarters, 416th Theater Engineer Commend, Darien, IL, dated 28 July 2009, show the applicant was assigned to the 317th Engineer Company, 3rd Platoon, in Kankakee, IL.

4.  Orders 200-1017, issued by the Camp Atterbury Joint Maneuver Training Center, Edinburgh, IN, dated 19 July 2010, show he was reassigned to the 317th Engineer Company, 2nd Platoon, in Kankakee, IL.

5.  Information obtained from Wikipedia states Kankakee is a city in Kankakee County, IL.  Additionally, the only listing in the internet for "Kankakee" is in Illinois.

DISCUSSION AND CONCLUSIONS:

1.  Since Kankakee is located in Illinois and his reassignment orders show he was assigned to the 317th Engineer Company in Kankakee, IL prior to his entry on active duty on 3 October 2009, it would be appropriate to correct item 7a of his DD Form 214 for the period ending 2 August 2010 to show Kankakee, Illinois instead of Wisconsin.

2.  In view of the foregoing, the applicant's DD Form 214 should be corrected as indicated below.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by deleting from item 7a of his DD Form 214 for the period ending 2 August 2010 


the entry "Kankakee, Wisconsin" and replacing it with the entry "Kankakee, Illinois."
