THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:
1. Application for correction of military records (with supporting documents provided, if any).
2. Military Personnel Records and advisory opinions (if any).
THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests, in effect, correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Combat Medical Badge, Army Commendation Medal, and awards for his service in Iraq.
2. He states these awards were omitted from his DD Form 214.
3. He provides:
* Combat Medical Badge orders
* DA Form 638 (Recommendation for Award) for the Army Commendation Medal
* DD Form 214
CONSIDERATION OF EVIDENCE:
1. The applicants military records show he enlisted in the Regular Army on 19 September 2005 for 4 years. He completed training and he was awarded military occupational specialty 68W, Health Care Specialist.
2. He provided Permanent Orders Number 102-039, issued by Headquarters, Joint Task Force on 11 April 2008, awarding him the Combat Medical Badge (1st
Award) for performance of duty while under hostile fire from 1 May 2007 to 14 March 2008.
3. He also provided a DA Form 638 which was approved on 23 March 2008 by Headquarters, 75th Ranger Regiment, Fort Benning, GA, awarding him the Army Commendation Medal for exceptionally meritorious achievement on 10 February 2008, by Permanent Order Number 083-29. His records are void of the award certificate.
4. He was honorably released from active duty on 18 September 2009 and was transferred to the U.S. Army Reserve Control Group. His DD Form 214 shows the following entries:
a. Item 12f (Foreign Service) does not show completion of any foreign service.
b. Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) lists the Army Good Conduct Medal, National Defense Service Medal, Army Service Ribbon, and the Parachutist Badge.
c. Item 18 (Remarks) does not show service in a hostile fire pay/imminent danger pay (HFP/IDP) area.
5. An email, dated 24 November 2010, from the Army Military Pay Operations, Defense Finance and Accounting Service (DFAS), confirmed he received HFP/IDP for service in a combat zone in Iraq from 6 January to 2 March 2008.
6. Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214. Chapter 2 contains guidance on the preparation of the DD Form 214. It states, in pertinent part, that the total amount of foreign service completed during the period covered by the DD Form 214 is entered in item 12f and is obtained from the Soldiers Enlisted/Officer Record Brief. Additionally, item 18 of the Soldier's DD Form 214 is used for mandatory requirements when a separate block is not available. For an active duty Soldier deployed with his or her unit during their continuous period of active service, enter the statement "SERVICE IN (NAME OF COUNTY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD-YYYYMMDD)."
7. Army Regulation 600-8-22 states the Iraq Campaign Medal is awarded to members who served in direct support of Operation Iraqi Freedom. The area of eligibility encompasses all the land area of the country of Iraq, the contiguous water area out to 12 nautical miles, and all air spaces above the land area of Iraq and above the contiguous water area out to 12 nautical miles. The Iraq Campaign Medal period of eligibility is on or after 19 March 2003 to a future date to be determined by the Secretary of Defense or the cessation of Operation Iraqi Freedom. Approved campaigns are:
* Liberation of Iraq (19 March 2003-1 May 2003)
* Transition of Iraq (2 May 2003-28 June 2004)
* Iraqi Governance (29 June 2004-15 December 2005)
* National Resolution (16 December 2005-9 January 2007)
* Iraqi Surge (10 January 2007-31 December 2008)
* Iraqi Sovereignty (1 January 2009-date to be determined)
8. Army Regulation 600-8-22 also states the Global War on Terrorism Service Medal is authorized for award to members of the Armed Forces of the United States who have participated in the Global War on Terrorism operations outside of the designated areas of eligibility on or after 11 September 2001 to a future date to be determined. Service members must be assigned, attached, or mobilized to a unit supporting designated operations for 30 consecutive days or for 60 nonconsecutive days.
DISCUSSION AND CONCLUSIONS:
1. The evidence of record confirms the applicant was awarded the Combat Medical Badge and Army Commendation Medal and these awards are not shown on his DD Form 214. Therefore, it would be appropriate to correct his DD Form 214 to show these awards.
2. The evidence of record confirms he completed 1 month and 25 days of foreign service in Kuwait/Iraq from 6 January through 2 March 2008. Therefore, he is entitled to correction of his DD Form 214 to show this period of foreign service.
3. In view of the foregoing, his records should be corrected as recommended below.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined the evidence presented is sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:
a. deleting from item 12f of his DD Form 214 the entry "0000 00 00" and adding the entry "0000 02 03";
b. adding to Item 13 of his DD Form 214 the Combat Medical Badge, Army Commendation Medal, Iraq Campaign Medal with one bronze service star, and Global War on Terrorism Service Medal;
c. adding to item 18 of his DD Form 214 the entry "Service in Kuwait/Iraq 20080106 - 20080302";
and
d. providing him a document that includes the above corrections.
