THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests reconsideration of his previous request to correct his DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) to show his social security number (SSN) as "XX7-40-XXXX" instead of "XX2-01-XXXX."

2.  The applicant states the first time his case was considered he was unable to locate his original social security card.

3.  The applicant provides copies of the following:

* his social security card, reissued on 27 November 2009
* his DD Form 215 (Correction to DD Form 214, Certificate of Release or Discharge from Active Duty)

CONSIDERATION OF EVIDENCE:

1.  Incorporated herein by reference are military records which were summarized in the previous consideration of the applicant's case by the Army Board for Correction of Military Records (ABCMR) in Docket Number AR20080019542 on 30 April 2009.

2.  The applicant's records show he was inducted into the Army of the United States on 20 December 1965.  A copy of his DD Form 47 (Record of Induction) lists his service number, but not his SSN.

3.  On 28 August 1967, the applicant was issued a DA Form 873 (Certificate of Clearance and/or Security Determination under Executive Order 10450) that shows his SSN as "XX7-40-XXXX."

4.  His DD Form 214 shows he was honorably released from active duty and transferred to the U.S. Army Reserve (USAR) Control Group (Annual Training) effective 13 December 1967.  Item 3 (Social Security Number) of this form shows his SSN as "XX2-01-XXXX."

5.  His records show that on 24 February 1968 the U.S. Army Reserve Components Personnel and Administration Center (RCPAC), St. Louis, MO, mailed the applicant a USAAC Form 1790 (Army Reserve Personnel Data Review) and asked him to make corrections as appropriate.  Accordingly, on 22 March 1968 the applicant made a correction to his SSN on this form to show his SSN as "XX7-40-XXXX" instead of "XX2-01-XXXX."

6.  On 8 December 1971, RCPAC published Letter Orders Number 12-1309274 honorably discharging the applicant from the USAR effective 19 December 1971 for completion of his military service obligations.  The orders list the applicant's SSN as "XX7-40-XXXX."

7.  The applicant provided a copy of his social security card which lists his SSN as "XX7-40-XXXX."

DISCUSSION AND CONCLUSIONS:

1.  The applicant's request for reconsideration to correct his SSN was carefully considered and found to have merit.

2.  The applicant's records contain documents which show his SSN as 
"XX7-40-XXXX."  It appears that his DD Form 214 erroneously listed his SSN as "XX2-01-XXXX."

3.  His records show his SSN was corrected by RCPAC prior to his discharge from the USAR.

4.  In view of the foregoing, the applicant's DD Form 214 should be corrected to show the SSN "XX7-40-XXXX."

BOARD DETERMINATION/RECOMMENDATION:

The Board determined the evidence presented is sufficient to warrant amendment of the ABCMR's decision in Docket Number AR20080019542, dated 30 April 2009.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by deleting the SSN 
"XX2-01-XXXX" from item 3 of the applicant's DD Form 214 for the period ending 13 December 1967 and replacing it with the SSN "XX7-40-XXXX."
