THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that his records be corrected to show his social security number (SSN) is 484-XX-1382.

2.  The applicant states his SSN is 484-XX-1382 and his brother’s SSN is 
478-XX-7118.  He states he used SSN 478-XX-7118 from 4 March 1999 until 
5 May 2009.  He needs all of his Army records corrected.  He states the Iowa Social Security Office put the wrong names on the social security cards.  He states his name is spelled Kaiphet P________ and he is serving in an active Army status, in pay grade E-6 and his brother’s name is spelled Kaiphat P________, he is a Reservist serving in pay grade of E-4.

3.  The applicant provides letters from the Social Security Administration in support of his application.  

CONSIDERATION OF EVIDENCE:

1.  The applicant is currently serving on active duty in the rank of staff sergeant.  

2.  The applicant enlisted in the Regular Army on 4 March 1999.  His DD Form 4/1 (Enlistment/Reenlistment Document – Armed Forces of the United States) shows his SSN as 478-XX-7118.  He was discharged on 4 October 2001 for immediate reenlistment.  He has served without a break in service reenlisting on 5 October 2001, 10 August 2004, and 13 August 2008.  His reenlistment contracts show his SSN as 478-XX-7118.

3.  A review of the applicant’s military personnel records in the Interactive Personnel Electronic Records Management System (IPERMS) show his SSN as 484-XX-1382, handwritten at the top of each document.  

4.  On the Social Security Administration, Statement of Claimant or Other Person, the applicant attested he has used his brother’s SSN of 478-XX-7118 and his brother has used his SSN of 484-XX-1382.  

5.  The letter from the Social Security Administration shows SSN 478-XX-7118 was assigned to the applicant’s brother, Kaiphat.  

DISCUSSION AND CONCLUSIONS:

1.  The applicant’s contentions regarding his SSN are acknowledged.  His service record contains sufficient evidence on which to grant the relief requested.   

2.  Documents from the Social Security Administration confirm the applicant’s SSN is 484-XX-1382 and his brother’s SSN is 478-XX-7118.

3.  The Statement of Claimant or Other Person certifies the applicant used his brother’s SSN of 478-XX-7118 during his tenure on active duty.  

4.  The applicant’s military personnel records in IPERMS show his SSN as 
478-XX-7118.  However, his SSN of 484-XX-1382 is handwritten at the top of those documents.   

5.  Since the evidence of record confirms the applicant has a brother using the same SSN of 478-XX-7118, it would be appropriate to correct his records to show his correct SSN of 484-XX-1382.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by changing his records to show his SSN is 484-XX-1382.
