THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 28 April 2009 to show the Army Commendation Medal, Army Service Ribbon, Global War on Terrorism Service Medal (GWOTSM), and National Defense Service Medal (NDSM).
2. The applicant states:
* the Army Commendation Medal is not listed on her DD Form 214
* orders show she was ordered to active duty for the purpose of operational support in support of Operation Enduring Freedom
3. The applicant provides:
* active duty orders
* Army Commendation Medal Certificate
* DD Form 214 for the period ending 28 April 2009
CONSIDERATION OF EVIDENCE:
1. The applicant was appointed as a Reserve commissioned officer in the U.S. Army Reserve (USAR) Veterinary Corps in the rank of first lieutenant on 12 November 2002. She was ordered to active duty from the USAR on 31 October 2008 for a period of 180 days and was released from active duty on 28 April 2009.
2. The applicant's DD Form 214 for the period ending 28 April 2009 shows the NDSM as an authorized award. Since her DD Form 214 shows the NDSM as an authorized award, this portion of her request will not be discussed further in these proceedings.
3. The applicant provided a copy of an Army Commendation Medal Certificate which shows she was awarded the Army Commendation Medal for the period 31 October 2008 to 28 April 2009 by the Great Plains Regional Veterinary Command, Permanent Order Number 104-01, dated 14 April 2009,
4. Army Regulation 600-8-22 (Military Awards) states the Army Service Ribbon was established by the Secretary of the Army on 10 April 1981. Effective 1 August 1981, all members of the Active Army, Army National Guard, and USAR in an active Reserve status are eligible for the award upon successful completion of initial entry training.
5. Army Regulation 600-8-22 states the GWOTSM is authorized for award to members of the Armed Forces of the United States who have participated in the Global War on Terrorism operations outside of the designated areas of eligibility on or after 11 September 2001 to a future date to be determined. All Soldiers on active duty, including Reserve Component Soldiers mobilized or National Guard Soldiers activated, on or after 11 September 2001 to a date to be determined having served 30 consecutive days or 60 nonconsecutive days are authorized the GWOTSM.
DISCUSSION AND CONCLUSIONS:
1. Orders show the applicant received the Army Commendation Medal. Therefore, her DD Form 214 for the period ending 28 April 2009 should be corrected to show this medal.
2. Since the applicant had an active Reserve status after 1 August 1981 and it is reasonable to presume she completed initial entry training, she is eligible for award of the Army Service Ribbon. Therefore, her DD Form 214 for the period ending 28 April 2009 should be corrected to show this ribbon.
3. The applicant served a period of qualifying service for award of the GWOTSM. Therefore, her DD Form 214 for the period ending 28 April 2009 should be corrected to show this medal.

BOARD DETERMINATION/RECOMMENDATION:
