THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests, in effect, correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to add his award of the Meritorious Service Medal (MSM).

2.  The applicant does not make any statement.

3.  The applicant provides:

* A copy of his DD Form 214
* A DA Form 4980-12 (The Meritorious Service Medal Certificate), issued by Headquarters, 9th Mission Support Command, Fort Shafter Flats, HI, dated 22 June 2009
* A DA Form 638 (Recommendation for Award), dated 22 June 2009

CONSIDERATION OF EVIDENCE:

1.  The applicant retired in the rank/grade of sergeant first class (SFC)/E-7 after completing 20 years and 24 days of creditable active Federal service.  His chain of command recognized his exceptionally meritorious service with award of the MSM.  Part V (Orders Data) of the DA Form 638 pertaining to the applicant's recommendation for award of the MSM shows this award was approved by a general officer under Permanent Order Number 173-03, by Headquarters,
9th Mission Support Command, Fort Shafter, dated 22 June 2009.

2.  Item 24 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) of his DD Form 214 does not show award of the MSM.

3.  Army Regulation 635-5 (Separation Documents) prescribes the separation documents that must be prepared for Soldiers on retirement, discharge, release from active duty service, or control of the Active Army.  It establishes standardized policy for preparing and distributing the DD Form 214.  It states that awards and decorations for all periods of service in the priority sequence specified in Army Regulation 600-8-22 (Military Awards) will be entered on the DD Form 214.  Each entry will be verified by the Soldier’s records.

DISCUSSION AND CONCLUSIONS:

The applicant was awarded the MSM as a retirement award, but it was not entered on the DD Form 214.  Therefore, his DD Form 214 should be corrected to show this award

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding to item 13 of his DD Form 214 the Meritorious Service Medal.
