THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of the last name shown on his DD Form 4 series (Enlistment/Reenlistment Document Armed Forces of the United States) and DD Form 1966 series (Record of Military Processing – Armed Forces of the United States).

2.  The applicant states that his last name is incorrect as shown on these documents.

3.  The applicant provides a copy of his Certificate of Live Birth.

CONSIDERATION OF EVIDENCE:

1.  On 26 February 2009, he enlisted in the Army Reserve using an unhyphenated last name.  He used the same name when he enlisted in the Regular Army (RA) on 15 April 2009.

2.  The last name he was using at the time of his enlistment is shown on his DD Form 4 series and his DD Form 1966 series.

3.  He submits a Certificate of Live Birth which shows his last name as it is reflected in item 1.b. (NAME) on his DD Form 149 (Application for Correction of Military Records).  His last name is hyphenated.

4.  The applicant is currently on active duty in the RA.

DISCUSSION AND CONCLUSIONS:

1.  The applicant contends the last name shown on his DD Form 4 series and DD Form 1966 series should be corrected.

2.  His contentions are noted and supported by the available evidence.  The last name on his Certificate of Live Birth is shown exactly as it is shown in item 1.b. on the DD Form 149 he submitted to this Board.

3.  The last name shown on his DD Form 4 series and DD Form 1966 series is incorrect and should be corrected to show his proper last name.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending his DD Form 4 series and his DD Form 1966 series to show the hyphenated last name reflected in item 1.b. on his DD Form 149.
