THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show his service in Iraq. 

2.  The applicant states, in effect, that his tour of duty in Iraq is not reflected on his DD Form 214. 

3.  The applicant provides, in support of his application, copies of his Soldier Deployment History Outprocessing Report and his Enlisted Record Brief.  

CONSIDERATION OF EVIDENCE:

1.  On 8 June 2006, the applicant enlisted in the Regular Army in the rank of specialist, pay grade E-4.  He completed his initial training and was awarded military occupational specialty (MOS) 68W (Health Care Specialist).  

2.  The applicant's Soldier Deployment History Report, dated 6 January 2009, shows the applicant was deployed from 6 June 2007 through 15 July 2008.

3.  The applicant's Enlisted Record Brief, dated 3 December 2008, shows that he applicant was awarded the Iraq Campaign Medal with bronze service star and an Overseas Service Ribbon.



4.  On 8 January 2009, the applicant was honorably discharged.  He had completed 2 years, 7 months, and 1 day of creditable active duty service.

5.  Item 12f (Foreign Service) of his DD Form 214 does not show any overseas service.

6.  Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) of the applicant's DD Form 214 lists his awards as the National Defense Service Medal, Global War on Terrorism Service Medal and the Army Service Ribbon.  

7.  Item 18 (Remarks) of the applicant's DD Form 214 does not include any remarks regarding his deployed service to Iraq.

8.  Army Regulation 635-5 (Separation Documents) provides detailed instructions for completing separation documents, including the DD Form 214.  It provides, in pertinent part, for an entry in block 12f of the total amount of foreign service completed during the current period of active duty.  It further provides that Item 18 will contain the following entry for Regular Army Soldiers: "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD - YYYYMMDD)." 

DISCUSSION AND CONCLUSIONS:

1.  The applicant contends that his service in Iraq is not reflected on his DD Form 214.

2.  The available evidence shows that the applicant served in Iraq from 
6 June 2007 to 15 July 2008 (1 year, 1 month, and 10 days).  This foreign service should be shown in Item 12f of his DD Form 214.

3.  Item 13 of the applicant's DD Form 214 should be corrected to add the Iraq Campaign Medal with one bronze service star and the Overseas Service Ribbon.

4.  Item 18 of the applicant's DD Form 214 should be corrected to show that he served in Iraq from 6 June 2007 to 15 July 2008.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

a. deleting the entry in Item 12f of his DD Form 214 and adding the entry
1 year, 1 month, and 10 days;

	b.  adding in Item 13 of his DD Form 214 the Iraq Campaign Medal with one bronze service star and the Overseas Service Ribbon; and

	c.  adding in Item 18 of his DD Form 214 "SERVICE IN IRAQ FROM 20070606 - 20080715."
