THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Army Superior Unit Award.

2.  The applicant states that he was a Staff Plastic Surgeon at Walter Reed Army Medical Center in 1991 when it was awarded the Army Superior Unit Award.  

3.  The applicant provides no documentation in support of his application.  

CONSIDERATION OF EVIDENCE:

1.  At the time of his application, the applicant was a retired Colonel, United States Army.

2.  On 21 January 1991, the applicant was assigned to Walter Reed Army Medical Center for duty as a plastic surgeon. 

3.  Department of the Army General Orders Number 34, dated 30 December 1992, awarded the Army Superior Unit Award to Walter Reed Army Medical Center for the period from 2 August 1990 to 30 June 1991.

4.  On 30 June 1995, the applicant was retired due to length of service.  A 
 DD Form 215 was issued on 2 December 1999 to correct his date of separation and record of service.  A subsequent DD Form 215 was issued on 17 August 2005 to correct his rank, pay grade and effective date of pay grade. 

5.  Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) of the applicant's DD Form 214 list his awards as the Meritorious Service medal, Meritorious Service Medal, Army Commendation Medal with Oak Leaf Cluster, National Defense Service Medal (two awards), Armed Forces Reserve Medal, Army Service Ribbon, Parachutist Badge, and Air Assault Badge.  It does not show award of the Army Superior Unit Award.

6.  Army Regulation 635-5 (Separation Documents) provides that when two 
DD Forms 215 have been issued and an additional correction is required, a new DD Form 214 will be issued.  There are no regulatory provisions for voiding a 
DD Form 215 to circumvent the issuance of a new DD Form 214.

DISCUSSION AND CONCLUSIONS:

1.  The applicant contends that his DD Form 214 should show his award of the Army Superior Unit Award.

2.  The available evidence shows that the applicant's unit was awarded the Army Superior Unit Award during the time of his assignment to the unit.  Therefore, his DD Form 214 should show this award.

3.  In view of the above, the applicant's request should be granted.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by issuing the applicant a new DD Form 214 incorporating the previous corrections and adding the Army Superior Unit Award.
