THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that his date of birth (DOB) be changed from 31 August 1970 to 31 August 1950 on his DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge).  He also requests that his blood type be changed from O+ to A+.  

2.  The applicant states that he would have been one year old according to his DD Form 214 when he was inducted.  

3.  The applicant provides a copy of his driver's license and his DD Form 214 in support of his application.  

CONSIDERATION OF EVIDENCE:

1.  The applicant underwent a pre-induction physical examination on 22 September 1969 which shows his DOB as 31 August 1950.  

2.  The applicant was inducted into the Army of the United States on 17 February 1970.  His DD Form 47 (Record of Induction) shows his DOB as 31 August 1950.

3.  The applicant's service personnel records contain a DD Form 398 (Statement of Personal History), dated 2 February 1970, which shows his DOB as 31 August 1950.  His DA Form 20 (Enlisted Qualification Record) shows his DOB as 31 August 1950.

4.  His Standard Form 601 (Immunization Record) shows his blood type as O+ and his DOB as 31 August 1950.

5.  The applicant underwent a separation physical on 26 October 1971 and his Standard Form 88 (Report of Medical Examination) shows his blood type as A+ and his DOB as 31 August 1950.  

6.  The applicant was honorably released from active duty on 8 January 1972.  

7.  Item 9 (DOB) on his DD Form 214 shows his DOB as 31 August 1970.

8.  Item 30 (Remarks) on his DD Form 214 shows his blood group as O+.

DISCUSSION AND CONCLUSIONS:

1.  Several documents in the applicant's service record confirm his DOB as 31 August 1950.  However, his DD Form 214 incorrectly shows his DOB as 31 August 1970.  Therefore, it would be appropriate to correct the applicant's DD Form 214 to show his DOB as 31 August 1950.

2.  Although the applicant's Immunization Record shows his blood type as O+, his Standard Form 88 indicates his blood type as A+.  His separation physical is accepted as sufficient evidence on which to base correction of his DD Form 214 to show his blood type as A+.  Therefore, item 30 of his DD Form 214 should be amended to reflect his blood type as A+.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that the DD Form 214 of the individual concerned be corrected by:

	a.  amending item 9 to show his DOB as 31 August 1950; and
	b.  amending item 30 to show his blood type as A+.
