THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests, in effect, correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 25 February 2006 to show award of the Good Conduct Medal and the Combat Action Badge.

2.  The applicant states, in effect, that these awards are not listed on this DD Form 214.

3.  The applicant provides orders for the Good Conduct Medal and the Combat Action Badge; reassignment orders; and a copy of his DD Form 214 for the period ending 25 February 2006 in support of his application.

CONSIDERATION OF EVIDENCE:

1.  The applicant was ordered to active duty from the U. S. Army Reserve on      3 January 2004 in support of Operation Iraqi Freedom.  On 25 February 2006, he was honorably discharged due to a physical disability.

2.  The applicant's DD Form 214 for the period ending 25 February 2006 does not show the Good Conduct Medal or the Combat Action Badge as authorized awards.

3.  Headquarters, Headquarters Company, 373rd Combat Sustainment Support Battalion Permanent Orders 88-52, dated 29 March 2006, show the applicant received the first award of the Good Conduct Medal for the period 3 January 2004 to 6 April 2005.
4.  Headquarters, United States Army 90th Regional Readiness Command Permanent Orders 153-30, dated 30 June 2006, show the applicant received the Combat Action Badge.

DISCUSSION AND CONCLUSIONS:

Orders show the applicant received the Good Conduct Medal and the Combat Action Badge.  Therefore, his DD Form 214 for the period ending 25 February 2006 should be corrected to show these awards.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding the Good Conduct Medal and the Combat Action Badge on his DD Form 214 for the period ending 25 February 2006.
