THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Purple Heart, the Combat Infantryman Badge, and the Army Commendation Medal.
2. The applicant states, in effect, that he was awarded the Purple Heart for wounds received in action on 26 June 2006, that he was issued the Combat Infantryman Badge while assigned/attached to an infantry unit engaged in actual combat, and that he was recommended for and authorized the Army Commendation Medal.
3. The applicant provides orders for the Purple Heart and the Combat Infantryman Badge; a letter, dated 1 August 2008, from the Bannock County Veterans Services in Pocatello, Idaho; and a copy of his DD Form 214.
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Regular Army on 31 August 2004. He served as an infantryman in Kuwait and Iraq and was honorably discharged on 28 July 2007 by reason of permanent disability.
2. The applicantÂ’s DD Form 214 does not show the Purple Heart, the Combat Infantryman Badge, or the Army Commendation Medal as authorized awards.
3. Headquarters, Landstuhl Regional Medical Center Permanent Order 187-002, dated 6 July 2006, shows the applicant received the Purple Heart for wounds received in action on 26 June 2006.
4. Headquarters, Multi-National Corps Iraq Permanent Orders 316-071, dated
12 November 2006, show the applicant received the Combat Infantryman Badge.
5. The applicantÂ’s records contain an Army Commendation Medal certificate awarded to the applicant for meritorious service during the period 17 November 2005 to 16 November 2006 during Operation Iraqi Freedom. This award was authorized based on Permanent Order Number 277, published by Headquarters, 1st Brigade, dated 1 October 2006.
DISCUSSION AND CONCLUSIONS:
Orders show the applicant received the Purple Heart, the Combat Infantryman Badge, and the Army Commendation Medal. Therefore, the applicant's DD Form 214 should be corrected to add these awards.
