THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:
1. Application for correction of military records (with supporting documents provided, if any).
2. Military Personnel Records and advisory opinions (if any).
THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests, in effect, correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show his dates of deployment during Operations Desert Shield/Desert Storm.
2. The applicant states that due to an administrative error his deployment dates in support of Desert Shield/Desert Storm were omitted from his DD Form 214.
3. The applicant provides, in support of his application, copies of his DD Form 214, unit mobilization orders, and a Certificate of Commendation for service among the coalition forces.
CONSIDERATION OF EVIDENCE:
1. On 1 June 1984, the applicant was ordered to active duty from the U. S. Army Reserve in an Active Guard Reserve (AGR) status, in the rank of staff sergeant, pay grade E-6.
2. Permanent Orders 160-6, First United States Army, dated 21 December 1990, as amended by Permanent Orders 161-7 with a roster and the applicants name listed on the roster, same command, dated 22 December 1990, mobilized the applicant's unit for a period of 180 days in support of Operation Desert Shield. The unit movement was to commence no later than 6 January 1991.
3. On 31 January 2008, the applicant was retired due to length of service. He had attained the rank of sergeant first class and had completed 23 years and
8 months of creditable active duty service as an AGR Soldier.
4. Item 12f (Foreign Service) of the applicant's DD Form 214 shows no overseas service during this period of active duty service. Item 18 (Remarks) of this same form does not show his period of mobilization and deployment in 1991.
5. The Certificate of Commendation, provided by the applicant, indicates that he served with distinction during Operation Desert Storm as a member of the coalition forces fighting for the liberation of Kuwait.
6. The Defense Finance and Accounting Service records show that the applicant served in Kuwait during the period from 1 to 30 June 1991.
7. The Gulf War Roster does not list the applicant's name.
8. Army Regulation 600-8-22 (Military Awards) provides at Appendix B, Table
B-1 for the following three campaign periods in the Persian Gulf War: Defense of Saudi Arabia (2 August 1991 - 16 January 1991); Liberation and Defense of Kuwait (17 January - 11 April 1991); and Southwest Asia Cease-Fire (12 April 1991 to 30 November 1995).
9. Army Regulation 635-5 (Separation Documents) provides detailed instructions for completing separation documents, including the DD Form 214. It provides, in pertinent part, for an entry in item 12f of the total amount of foreign service completed during the current period of active duty. It further provides that item 18 will contain the following entries for Reserve Army Soldiers: "Ordered to active duty in support of [name of operation] per 10 USC (applicable section)"; "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD - YYYYMMDD)"; and "Soldier completed (or did not complete) period for which ordered to active duty for the purpose of post-service benefits and entitlements."
10. Army Regulation 635-5 (Separation Documents) further provides in pertinent part, for an entry in block 12f of the total amount of foreign service completed during the current period of active duty.
DISCUSSION AND CONCLUSIONS:
1. The applicant contends that his DD Form 214 should be corrected to show his dates of deployment in support of Operation Desert Shield/Desert Storm.
2. The available evidence clearly shows that the applicant's Reserve unit was mobilized in support of Operation Desert Shield for a 180 days commencing on or about 6 January 1991. It further shows that the applicant served in Kuwait during the period from 1 to 30 June 1991.
3. In view of the above, the applicant's DD Form 214 should be corrected to show his deployed service.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:
a. deleting the entry in Item 12f of his DD Form 214 and adding the entry 0000 Years, 01 Month, 00 Days; and
b. adding in Item 18 of his DD Form 214: "Mobilized on or about 6 January 1991 in support of Operation Desert Shield for a period not to exceed 180 days" and "Served in Kuwait from 1 to 30 June 1991."
